狼疮性肾炎68例临床病理分析

摘要 目的探讨狼疮性肾炎患者的临床表现与病理类型的关系。方法对68例狼疮性肾炎(LN)患者行肾活检,进行临床与病理分析。结果各病理类型中,Ⅳ型所占比例最多,其次为Ⅱ型、Ⅴ型。Ⅱ型、Ⅲ型主要表现为蛋白尿,且Ⅲ型患者血尿重于Ⅱ型。Ⅳ型和Ⅴ+Ⅳ型高血压、血尿和肾功能不全发生率均最高。Ⅴ+Ⅲ型与Ⅴ型蛋白尿均高于Ⅲ型,Ⅴ+Ⅲ型血尿重于Ⅴ型和Ⅲ型。Ⅳ型补体C3下降率、抗dsDNA阳性率及活动性指数(AI)明显高于其他类型,差异有统计学意义(P<0.05)。慢性指数(CI)以Ⅳ型及Ⅴ型为高,而肾小管间质病变(TIL)以Ⅲ型及Ⅳ型为重。结论Ⅳ型是最常见、活动性最高、临床表现最突出、预后最差的一种类型,需要积极治疗。狼疮性肾炎的病理类型与临床表现有一定相关性,所以临床表现、免疫学指标要和病理结合起来共同制订治疗方案和判断预后。
