摘要
目的评价帕金森病（PD）患者睡眠质量，探讨PD患者睡眠障碍的临床特征。方法采用帕金森病睡眠量表（parkinson disease sleep scale，PDSS）、爱泼沃斯思睡量表（epworth sleepiness scale，ESS）分别对30例早中期PD患者（Hoehn-Yahr分期Ⅰ～Ⅱ，PD1组）、19例中晚期PD患者（Hoehn-Yahr分期Ⅲ～Ⅴ，PD2组）和34例年龄、性别相匹配对照者的睡眠状况及临床特点进行评估。结果PDSS总评分，PD1组（116．00±16．97）和PD2组（96．84±21．08）明显低于对照组，与对照组（131．73±11．04）比较差异均有统计学意义（P〈0.01）。PDI组患者睡眠障碍的主要类型有入睡困难（36．67％）、睡眠维持困难（33．33％）、夜尿症（33．33％）、睡眠中烦躁（16．67％）、肢体痉挛性疼痛（13．33％）。PD2组除睡眠维持困难（57．89％）、烦躁（57．89％）、入睡困难（52．63％）、夜尿症（52．63％）、恶梦（42．11％）外，震颤（31．58％）、早醒（31．58％）、幻觉（26．31％）及日间过度思睡（47．37％）等症状均较突出。结论PD患者与对照组比较具有明显睡眠障碍，其临床主要表现为入睡困难和睡眠维持困难、夜尿症、睡眠行为障碍及白天过度嗜睡，中晚期患者临床症状更为明显。
Objective To evaluate the quality of sleep and investigate the clinical characteristics of sleep disorders in patients with Parkinson＇s disease （PD）. Methods Sleep assessment was performed by using the parkinson disease sleep scale （PDSS） and epworth sleepiness scale （ESS） sleep questionnaire. 30 PD patients at early stage （Hoehn-Yahr grade Ⅰ-Ⅱ ,PD1）, 19 PD patients at advanced stage （Hoehn-Yahr grade Ⅲ-Ⅴ ,PD2） and age-matched 34 healthy Controls participated in this study. Results The total PDSS score in PD1 group （116.00±16.97） and PD2 group（96.84±21.08）was significantly lower than that in control group （131.73±11.04） （P〈0.01）. The manifestation of sleep disorders in PD1 patients included： difficulties in the initiation of sleep （36.67%） ,fragmented sleep（33.33%）,nocturia （33.33%） ,fidget in bed （16.67%） ,and painful muscle cramps （13.33%）. The main type of sleep disorders in PD2 patinets was fragmented sleep （57. 89%）, fidget in bed （57. 89%）, initiation of sleep （52.63%）,nocturia （52.63%）,nightmares （42. 11%）,tremor （31. 58%）,waking early （31. 58%）,hallucination（26.31%） and excessive daytime sleepiness （47.37%）. Conclusion Sleep disorders are more common in PD patients than in controls. The types of sleep disorders in patients with PD include difficulties in the initiation of sleep, fragmented sleep, nocturia, sleep behavior disorders, and excessive daytime sleepiness. PD patients at advanced stage have more severity of sleep disorder than that at early stage.
