摘要
目的:了解宫颈液基细胞学检查(thinp rep cytologic test,TCT)与高危型人乳头瘤病毒(human papillomavirus,HPV)检测在宫颈癌早期筛查中癌前病变检出率的不同,探讨将两者联合运用于宫颈癌及癌前病变早期筛查中的临床价值。方法:对2007年9月至2008年8月于本院就诊的1000例患者同时进行宫颈液基细胞学检查与高危型HPV检测,对宫颈液基细胞学检查和(或)高危型HPV检测发现异常的患者在阴道镜下行宫颈组织病理学检查,并与同期门诊仅行宫颈液基细胞学检查的750例患者对比。结果:1000例患者中筛查出71例高危型HPV感染者,感染率为7.10%;TCT涂片异常者64例,占6.40%;两者均为阳性者35例,为3.50%。高危型HPV检测联合宫颈液基细胞学检查与单纯宫颈液基细胞学检查比较,两者中低度鳞状上皮内病变(LSIL)分别为2.10%和0.67%,差异有显著性意义(P<0.05);高度鳞状上皮内病变(HSIL)为0.90%和0.27%,癌为0.30%和0.13%,差异无显著性意义(P>0.05)。结论:高危型HPV检测技术可降低宫颈液基细胞学检查技术的假阴性率,将宫颈液基细胞学检查与高危型HPV检测技术联合进行宫颈癌及癌前病变早期筛查,其筛查效率高于单独使用宫颈液基细胞学检查技术。
Objective：To understand the difference between the thinprep cytologic test and the high - risk human papillomavirus technology screening in the cervical cancer precursor, and evaluate diagnostic value of the thinprep cytologic united the high - risk human papillomavirus test screening in the cervical cancer precursor. Methods： A comparative study was carried out on the TCT and HR - HPV in the diagnosis of the cervical cancer precursor, by experimenting with 1000 people who had been examined by the TCT and HR - HPV. At the same time ,to compare with 750 people who had been examined by the TCT froom September 2007 to August 2008 in Wen zhou Municipal Lu Cheng Region People Hospital. Unusual results had been pathologically diagnosed by colposcope. Results：The masculinere suit ratio of the HR - HPV was 7. 10% （71/1000） ; The masculineresult ratio of the TCT was 6.40% （64/1000）. The masculineresult ratio of the HR -HPV and the TCT was 3. 50% （35/1000）. The comparison of diag- nosis in differenct levels was as fllows：LSIL（ low- grade squamous intraepitnelial lesion） ：The HR- HPV united the TCTand the TCT were with significant difference（ P 〈0.05 ） ;HSIL（ high - grade squamous intraepitnelial lesion） ： The HR -HPV united the TCTand the TCTwere with no difference （ P 〉 0. 05 ）. Conclusion： The HR - HPV test can reduce the false negative ratio of the TCT. So in screening of cervical lesions, the ratio of the TCT united the HR - HPV test will be higher than the TCT.
