MRI和MRS对前列腺疾病的诊断及鉴别诊断价值 被引量:3

摘要 目的探讨磁共振成像(MRI)和磁共振波谱分析(MRS)在前列腺癌(PC)的诊断及鉴别诊断中的价值。方法对经病理穿刺活检、手术及随访证实的36例PC和68例前列腺增生(BPH)患者,进行常规MRI和MRS扫描。MRI观察前列腺大小、病变位置、信号特点及肿瘤侵犯情况。MRS观察枸缘酸盐(Cit)、胆碱复合物(Cho)、肌酸(Cr)的波峰与(Cho+Cr)/Cit值。结果MRI显示36例PC患者中,发生于外周带23例,表现为T2WI外周带内异常低信号区或前列腺带状结构破坏,周围带与中央带界线消失。7例位于包膜内,25例突破包膜侵犯精囊或血管神经束,其中17例伴有盆腔淋巴结和(或)骨转移。MRS显示PC患者的Cit明显下降,Cho明显升高,(Cho+Cr)/Cit值为2.8±1.0;而BPH患者的Cit波峰明显升高,Cho波峰下降,病灶区平均(Cho+Cr)/Cit值为0.57±0.20。结论MRI和MRS在PC诊断中具有特异性表现,2种方法联合应用极大提高了MR对前列腺癌诊断的正确率,并对PC与BPH的鉴别诊断具有重要价值。
