摘要
目的:分析研究前列腺癌的螺旋CT表现,以提高其诊断准确率。方法:对本院2005年6月~2009年6月收治的41例活检或临床证实的前列腺癌患者进行螺旋CT平扫及增强扫描,层厚5mm,螺距1.0,注射造影剂20s后进行动脉期扫描,70s后进行静脉期扫描。结果:41例前列腺癌患者中,前列腺形态均发生改变,边缘局限性突起者占95.1%,癌症对周围组织侵犯者占85.4%,病灶转移者占60.9%,螺旋CT平扫,前列腺局限性密度降低或不均匀者占63.4%,增强扫描静脉期所有患者均呈现前列腺密度降低或不均匀,增强扫描中早期强化者占51.2%。结论:前列腺癌的螺旋CT诊断最主要表现为动态扫描中前列腺密度降低或不均匀,伴随有形态变化、周围组织侵犯及增强扫描的部分早期强化,该诊断方法在前列腺癌的诊断及鉴别诊断中具有一定的应用价值和临床意义。
Objective: To analyze the performance of spiral CT for prostate cancer, to improve its diagnostic accuracy; Methods: From June 2005 to June 2009 41 prostate cancer patients with biopsy or clinical confirmation got spiral plain and enhanced CT scans, slice thickness 5 mm, pitch 1.0, 20 s after injection of contrast medium for arterial phase scanning, 70 s after the venous phase scan. Results: 41 patients with prostate cancer, both the prostate shape changed, edge limitations of processes accounted for 95.1%, surrounding tissue invasion accounted for the 85.4%, metastasis lesion accounted for 60.9%, The limitations of reduced or non-uniform density of prostate accounted for 61.9%, of all cases enhanced scanning were intravenous showed reduced or uneven density of the prostate, early intensive enhancement scanning accounted for 51.2%. Conclusion: For diagnosis of prostate cancer, the most important performance of spiral CT scan of the prostate is the dynamic reduced or uneven density, accompanied by morphological changes, and enhanced peripheral tissue invasion early intensive part of the scan, the diagnosis of prostate cancer diagnosis and differential diagnosis has some value and clinical significance.
