摘要
目的研究快速刺激及药物(维拉帕米、阿托品)刺激对犬肺静脉电重构的影响,探讨肺静脉对心房颤动(房颤)的发生及维持的作用。方法测量快速刺激及药物(维拉帕米、阿托品)刺激犬肺静脉时肺静脉有效不应期、有效不应期频率适应性、房颤诱发率和持续时间。结果①与刺激前比较,快速刺激时肺静脉有效不应期及有效不应期频率适应性在刺激后即刻、5min有显著性差异,刺激后10min时无明显差异;刺激前、后肺静脉的房颤诱发率及房颤持续时间有显著性差异。②维拉帕米、阿托品使肺静脉有效不应期缩短及有效不应期频率适应性下降。结论快速刺激犬肺静脉可以导致肺静脉发生急性电重构,但可在短时间内恢复。在犬的试验中维拉帕米、阿托品可改变肺静脉发生电重构的程度,并加快肺静脉有效不应期频率适应性恢复速度。
Objective To investigate the influence of electrical remodeling for pulmonary vein on animal model and observe the effect of electrical remodeling by giving veripamil and atropine after rapid PV stimulation. Methods Basic PV-ERP was measured firstly on animal model. Next, the inducible ratio and duration of atrium flicker were observed after the inducement of atrial fibrillation which S1S1 burst stimulus and S1S2 programmed stimulus method were adopted. Finally, the effect of the PV-ERP was observed after giving veripamil and atropine. Results Significant difference was found in the observation for the ratio adaptability for PV-ERP and the PV-ERP after rapid stimulation at 0 min and 5 min, while no differentia at 10 min. There was also prominent differentia in the inducible ratio and duration of atrial fibrillation after rapid stimulation. The electrical remodeling for pulmonary vein was reversed by giving veripamil, the lever of the shorten PV-ERP were weakened, and frequency adaptability of PV-ERP were decreased and recovered quickly after giving atropine. Conclusions The phenomena of acute electrical remodeling resulted from the rapid stimulation for pulmonary vein and recovered rapidly in the observation. The electrical remodeling were reversed by giving veripamil. By giving atropine, the level of acute electrical remodeling were decreased and the agency of interdiction of pneumogastric nerve and the recovery of frequency adaptability for PV-ERP were accelerated.
