针刺效应影响因素的定性定量化实验研究 被引量:19

Qualitative and Quantitative Experiment Study on Influential Factors of Acupuncture Efficacy

摘要 目的:以氢化可的松造成肾阳虚实验动物模型为研究对象,采用正交设计研究影响针刺效应的多个因素,探索影响针刺效应的主次因素和针刺参数的最佳组合,以期为提高针刺疗效、指导针灸临床提供实验依据。方法:采用L8(27)正交表对4因素(穴位的相对特异性、针刺捻转角度和频率、针刺时间、针刺间隔时间)2水平安排实验,运用自行研制的计算机控制针刺手法仪治疗肾阳虚大鼠,通过观察睾酮、雌二醇的水平来比较上述4因素的主次。结果:针刺对肾阳虚大鼠都有一定的治疗作用(P<0.05或<0.01);上述4因素对针刺效应的影响大小依次是捻针角度和频率、穴位选择、针刺时间或针刺间隔时间,其中捻针角度和频率是影响针刺效应的主要因素或重要因素(P<0.01或<0.05);腧穴的选择也是影响针刺效应的重要因素(P均<0.05);而针刺时间和针刺间隔时间相对来说是次要因素(P均>0.05)。结论:①针刺对虚证具有"补"的作用;②针刺治疗肾阳虚大鼠的最佳针刺参数为选用肾俞穴、采用小角度慢频率的捻转、每天治疗1次、每次1m in;③本实验研究中,不同因素对针刺治疗肾阳虚大鼠的疗效不同,其中捻针的角度和频率对疗效的影响最大,其次为穴位的选用,再次为针刺时间和针刺间隔时间。 Objective: To explore first - secondary factors that influenced acupuncture efficacy and the best combination of acupuncture parameters, and to provide experimental evidence for elevating acupuncture efficacy and guiding acupuncture clinic, using orthogonal design to study several factors that influenced acupuncture efficacy, as the animal model of kidney - YANG deficiency made by hydrocortisone was the study object. Methods: To arrange experiment for studying 4 factors( relative specificity of acupomt, twisting--rotating comer and frequency of acupuncture, acupuncture time and interval time of acupuncture)and 2 levels by using Ls (27 ) orthogonal table, to treat kidney - YANG deficiency rats by using self - made acupuncture manipulation device controlled by computer, and to compare which was the first or secondary factor among 4 factors mentioned above. Results: There were certain efficacy in the treatment of kidney - YANG deficiency rats by acupuncture ( P 〈 0.05 or 〈 0.01 ). The sequence of acupuncture efficacy influenced by 4 factors mentioned above was : twisting - rotating comer and frequency, selection of acupoint, acupuncture time or acupuncture interval time; among them, twisting -rotating comer and frequency was the main or important factor influenced acupuncture efficacy( P 〈 0.01 or 〈 0.05) ,selection of acupoint also was the important factor influenced acupuncture efficacy ( P 〈 0.05) ,relatively acupuncture time and acupuncture interval time were secondary factor( P both 〉 0.05 ) . Conclusion :①Acupuncture has reinforcing action for the deficient syndrome;②The best acupuncture parameters for treating kidney - YANG deficiency rats were: selecting Shenshu ( B 23), using small comer and slow frequency when twist and rotate needle, one time per day, one minute each time;③In this study, different acupuncture parameter has different efficacy for kidney - YANG deficiency rats ; among them, twisting - rotating comer and frequency became the biggest influential factor, selection of acupoint was the secondary factor, and the last one was acupuncture time or acupuncture interval time.
