摘要
目的:探讨腹水VEGF、ADA及LDH对腹水性质的鉴别价值。方法:对20例肝硬化腹水、13例结核性腹水、22例癌性腹水VEGF、ADA及LDH含量进行检测,并同时检测此55例患者血清ADA的含量。结果:癌性腹水组VEGF(642.07±404.90pg/ml)明显高于肝硬化组(82.03±35.84 pg/ml)与结核组(84.96±35.66pg/ml)(P<0.01),肝硬化组与结核性腹水比较,差异无显著性(P>0.05)。结核性腹水组腹水ADA/血清ADA(PADA/SADA)为1.94±0.79,明显高于肝硬化腹水组0.86±0.17(P<0.01)和癌性腹水组0.93±0.08(P<0.01);结核组ADA为25.68±4.73 U/L,明显高于肝硬化腹水组7.15±4.14 U/L(P<0.01)和癌性腹水组12.48±6.17 U/L(P<0.01)。结核组与癌性组LDH分别为218.08±70.78 U/L和200.91±99.27U/L均明显高于肝硬化组60.75±35.27U/L(P<0.01)。结论:腹水VEGF是良恶性腹水重要的鉴别诊断依据之一;腹水ADA及PADA/SADA对诊断结核性腹水有重要的临床参考价值;腹水LDH对于鉴别腹水为漏出液还是渗出液有一定价值。
[Objective]To study the value of ADA,VEGF and LDH of ascites in the different diagnosis of the character of ascites.[Method]The levels of VEGF,ADA and LDH in ascites specimens from 13 tubercle,20 hepatocinhosis and 22 cancer ascites were tested.The levels of ADA in serum of the 55 cases were tested simultaneously.[Result]The level of ascites VEGF in cancer ascites was 42.07±404.90 pg/ml,which was significantly higher than that of the group of tubercule ascites (84.96±35.66 pg/ml) and that of the group of hepatocinhosis ascites(82.03±35.84pg/ml)(P < 0.01).There was no obvious difference between the group of hepatocinhosis ascites and the group of tubercular ascites(P > 0.05).The ratio of ADA in ascites to ADA in serum(PADA/SADA) in tubercular ascites(1.94±0.79) was higher than that in cancer ascites (0.93±0.08) and that of hepatocinhosis ascites(0.86±0.17)(P < 0.01).The level of ascites ADA in tubercle ascites (25.68±4.73 U/L) was significandy higher than that in cancer ascites(12.48±6.17 U/L) and hepatocinhosis ascites(7.15±4.14 U/L)(P < 0.01).The level of ascites LDH in tubercle ascites(218.08±70.78) U/L and cancer ascites(200.91±99.27) U/L were both higher than that in hepatocinhosis ascites(60.75±35.27) U/L(P < 0.01).[Conclusion]VEGF in ascites is one of the important basis to the diagnosis of benign and malignant ascites;ADA and PADA/SADAof ascites have important reference to diagnosing tubercular ascites;LDH has definite value to identify the character of ascites which is exuding liquid or leaking liquid.
