摘要
目的 探讨系统性红斑狼疮 (SL E)的发病机理和遗传基础。方法 分析了 SL E患者中各种自身抗体与 H L A-DR、DQ基因的关联。结果 发现抗 Ro抗体与 DR2、DQ6及 DQ2 /DQ6杂合子相关 ,抗RNP抗体与 DQA 1* 0 10 2、DQB1* 0 6 0 2相关。抗 Sm和抗 ds DN A抗体均与 DRB1* 15 0 1、DQ A1* 0 10 2、DQB1* 0 6 0 2相关 ,这与汉族 SL E与 HL A的关联完全一致 ,表明抗 Sm和抗 ds DN A抗体对 SL E有极高的诊断价值。结论 自身抗体与 H L A的关联跨越了种族界限 ,不同种族的 SL E患者中同种自身抗体与H L A抗原的关联基本相同 ,提示 HL A对 SL E的遗传易感作用主要发生在自身抗体水平。
Objective To study the pathogenesis and genetic basis of system ic lupus erythem atosus (SL E) .Methods The association between H L A-DR,DQ genes and various autoantibodies in patients w ith SL E was analysed. Results The results indicated that DR2 ,DQ 6 and the heterozygous com bination DQ2 / DQ6 were closely correlated w ith the production of autoantibodies to Ro antigen,and DQ A1* 0 10 2 , DQB1* 0 6 0 2 with antibody to RNP. As shown in the lupus population study in H an nationality,patients w ith anti-Sm and anti-ds DNA antibodies were associated w ith DRB1* 15 0 1,DQA1* 0 10 2 ,and DQB1* 0 6 0 2 which suggests that anti-Sm and anti-ds DN A antibodies have a very im portant diagnostic value in SL E.Conclusion The association betw een autoantibodies and HL A has gone beyond ethnic boundary.In various ethnic patients with SL E,the associaton between HL A antigen and specific autoantibody is sim i- lar.The above data suggest that contribution of class MHC m olecules in SL E is predom inantly at the level of specific autoantibodies production.
