合并右束支传导阻滞急性心肌梗死患者的冠脉病变特征和临床特点 被引量:1

摘要 目的:探讨合并右束支传导阻滞(RBBB)急性心肌梗死患者的冠脉病变特征和临床特点。方法:急性心肌梗死患者352例,按是否合并RBBB分为两组,其中RBBB组48例,非RBBB组304例,观察急性心肌梗死伴RBBB患者心肌梗死部位及梗死相关动脉的特点、RBBB组和非RBBB组间血清肌酸激酶(CK)和肌酸激酶同功酶(CK-MB)峰值、心功能K illip分级、严重心律失常发生率及住院期间病死率。结果:RBBB组梗死部位多为前壁或广泛前壁,梗死动脉多为前降支近端;RBBB组CK和CK-MB峰值、心功能K illip分级、恶性心律失常发生率、院内病死率均高于非RBBB组(均P<0.05)。结论:急性心肌梗死出现RBBB多提示前降支近端闭塞,梗死面积大,病情重,易发生心衰和恶性心律失常,预后差。
