WB-DWI结合常规CT、MRI诊断恶性肿瘤全身脏器转移 被引量:7

Diagnosis of Malignant Metastases by WB-DWI and CT/MRI

摘要 目的探讨联合应用全身磁共振扩散成像(WB-DWI)技术和CT/MRI在诊断恶性肿瘤全身转移上的临床应用价值。资料与方法选择经临床病理证实的全身恶性肿瘤患者34例,男20例,女14例,年龄22~75岁,平均年龄45岁。所有病例先行CT/MRI检查;而后行WB-DWI检查及后续CT/MRI增强扫描。对照手术、穿刺病理及临床随访结果,统计单独应用CT/MRI影像以及WB-DWI+CT/MRI综合影像对全身转移病灶进行诊断的敏感性、特异性和准确性。统计两者在诊断恶性肿瘤全身转移上的统计学差异。结果CT/MRI常规检查共诊断转移病灶61处,行WB-DWI+CT/MRI诊断转移病灶84处,病理及临床诊断转移病灶83处。CT/MRI常规检查诊断转移病灶符合率约66.37%,WB-DWI+CT/MRI诊断转移病灶符合率约89.11%,两者在诊断恶性肿瘤全身转移上差异具有统计学意义(P<0.05)。结论应用WB-DWI对恶性肿瘤全身转移进行诊断,具有明显的临床应用价值。 Objective To investigate the values of WB-DWI and CT/MRI in diagnosing the metastases from malignant tumors. Materials and Methods Thirty four patients (20 male and 14 female, with the average age of 45yr) with malignant tumors underwent routine CT/MRI and WB-DWI examinations. The accuracy, sensitivity and specificity of routine CT/MRI and WB-DWI + CT/MRI were calculated. Results There were 61 metastatic lesions detected by routine CT/ MRI, 84 by WB-DWI + CT/MRI, and 83 by pathological findings and clinical diagnosis. The accuracy were 66.37% by routine CT/MRI, 89.11% by WB-DWI + CT/MRI. There were significant differences in diagnosing malignant metastases between by routine CT/MRI and by WB-DWI + CT/MRI (P 〈 0.05). Conclusion WB-DWI would be a new important method to diagnose malignant metastases.
