静脉窦性脑梗死患者的临床与影像学观察 被引量:3

摘要 目的提高对静脉窦性脑梗死的认识和诊断水平。方法回顾性分析已确诊的脑静脉窦血栓形成患者209例,总结了其中17例脑静脉窦性脑梗死患者的临床表现和辅助检查的(包括影像学检查等)特点。结果静脉窦性脑梗死患者的临床表现、神经系统体征及脑脊液等辅助检查无特异性。头颅MRI表现水肿范围大,而梗死灶相对较小,时间上相对延迟;脑梗死部位不是按动脉走行分布,形状呈片状;脑梗死常位于皮质和皮质下较表浅部位;多伴有出血等特点。结论头颅MRI检查有助于诊断,但静脉窦性脑梗死确诊还需要数字减影血管造影术检查。 Objective To enhance the knowledge and diagnosis level on venous sinus cerebral infarction. Methods 209 cases who had been diagnosed as cerebral venous sinus thrombosis were analysed retrospectively. The features of clinical manifestations and auxiliary examination of 17 cases of venous sinus cerebral infarction were summarized. Results The symptoms,signs and examination results of cerebrospinal fluid in venous sinus cerebral infarction patients had no special features. The head MRI examination showed that the extent of brain edema was bigger than that of infarct size, brain edema appeared earlier than infarct, the site of infarct was not distributed along the arterial distribution, and the infarcts presented patch in shape. The site of infarct was frequently located in the cortex and subcortex. The infarct was mostly accompanied by cerebral hemorrhage. Conclusions The head MRI examination is helpful to the diagnosis of venous sinus cerebral infarction. DSA is needed for final diagnosis.
