经皮微创动力髋螺钉治疗不稳定股骨转子间骨折 被引量:2

Clinical analysis of minimally invasive percutaneous plate osteosynthesis with dynamic hip screw in treatment of unstable intertrochanteric femoral fracture

摘要 目的回顾性研究经皮微创动力髋螺钉(DHS)治疗不稳定股骨转子间骨折的效果.探讨该方法的治疗价值。方法根据骨折微循环保护理念采用间接复位.经有限的小切口对21例不稳定股骨转子间骨折患者行DHS治疗。术后早期进行主动髋膝关节屈伸练习.根据骨折愈合情况决定患肢负重时间。结果经过12~24个月(平均15.5个月)随访.患者骨折全部愈合.平均临床愈合时间10.4周。1例有6°髋内翻.2例有5°和7°髋外翻,2例有10°下肢外旋.2例有15mm下肢缩短。无手术部位感染、内固定折断及骨折再移位发生。根据Sanders髋部创伤评分标准.优良率为100%,所有患者达到受伤前运动水平。结论经皮微创DHS手术治疗合适类型不稳定股骨转子问骨折具有技术上的优势。DHS置入质量是患者术后髋关节功能恢复良好的保证。 Objective This retrospective study is to evaluate the therapeutic efficacy of dynamic hip screw (DHS) applied for unstable intertroehanteric femoral fracture by minimally invasive percutaneous plate osteosynthesis (MIPPO) and to assess the clinic value of tbis method. Methods According to the principles of conservation of the vascularity, 21 patients of comminuted intertrochanteric femoral fractures were treated with indirect reduction and stabilized by DHS with MIPPO technique. All of the pa tients were allowed to take on active motions for recovery at the early time postoperatively and mean time of full weight bearing was determined individually by the bony healing. Results During the follow up for 12-24 months with average of 15.5 months, union was achieved in all the patients without bone grafting. There were 1 varus deformity of 6°, 2 valgus deformity of 7°and 5°, 2 rotational deformity of 10°and 2 shortening of 15 mm. None cases of surgical site infection, implant failure and displacement of fracture were observed. According to the Sanders traumatic hip score rating system, functional results rated as excellent or good in 100.0%. All patients returned to their previous activity states. Conclusions With regard to suitable case, DHS used with MIPO possess of special advantages in technical aspects and the implant quality is the cornerstone for satisfactory functional recovery.
