摘要
目的:探讨板层角膜移植术对带状角膜病变的治疗效果。方法:2003-01/2008-01我院角膜病组收治带状角膜病变患者16例(16眼)。其中男12例(12眼),女4例(4眼),年龄20～55岁,病史3～20a,患眼视力为光感～0.1不等。手术方法:根据病变的范围,选择不同直径环钻切除病灶,10-0的尼龙线间断缝合等大的植片和植床。结果:术后1wk内,植片可轻度水肿,之后渐变清亮,术后观察6mo～5a,未见病变复发及发生排斥反应者。术后视力均有明显提高,术后6mo,视力≥0.5者3眼,0.1～0.5者11眼,指数～0.1者2眼。结论:板层角膜移植术可以控制带状角膜病变病情发展,提高患眼视力,远期疗效稳定,是治疗带状角膜病变安全有效的疗法。
AIM： To investigate the treatment effect of lamellar keratoplasty on band-shaped keratopathy.
METHODS： Sixteen cases （16 eyes） between January 2003 to January 2008, in which 12 cases （12 eyes） were male and 4 cases （4 eyes） were female, aged from 20 to 55 years old, were included. The time of the disease was between 3 and 20 years. The visual acuity was between light perception and 0. 1. According to the range of the disease, an annular-shaped ectatic lesion was defined and removed, and donor tissue of the same size and shape was sutured on the recipient bed with 10- 0 nylon.
RESULTS： There was a little edema in cornea in one week, and then became clarity. No rejections and complications occurred between 6 months and 5 years of follow up. The visual acuity improved obviously. After lamellar keratoplasty 6 months, the visual acuity was 〉 0.5 in 3 eyes, 0.1-0.5 in 11 eyes, and 〈0.1 in 2 eyes.
CONCLUSION： We have developed a safe, successful, and simple procedure for repairing the severe peripheral marginal ectasia in band-shaped keratopathy. This technique substantially improved visual acuity.
