The Change and Clinical Significances of Serum MMP-9 and Inflammatory Factor in Patients of Acute Organophosphorus Pesticide Poisoning with Multiple Organ Dysfunction Syndrome

摘要 目的观察急性有机磷农药中毒(AOPP)致多器官功能障碍综合征(MODS)患者血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-10(IL-10)及基质金属蛋白酶-9(MMP-9)的动态变化及其关系,探讨 AOPP 引发 MODS 的机制。方法采用酶联免疫吸附试验双抗体夹心法检测42例 AOPP 伴 MODS 患者血清中 TNF-α、IL-6、IL-10及 MMP-9的水平,与正常对照组进行比较,并采用 Pearson 相关分析方法分析 MMP-9与 TNF—α、IL-6、IL-10的相关关系。结果 AOPP 患者血清TNF-α、IL-6、IL-10含量均升高,与正常对照组相比差异非常显著。血清 MMP-9水平与 TNF-α、IL-6、IL-10的水平呈明显的正相关关系。结论 AOPP 可导致炎症递质失调。MMP-9通过调控炎症递质参与了全身炎症反应综合征(SIRS)向 MODS 的发生发展过程。 Objective To investigate the dynamic variations and relationship of tumor necrosis factor-α(TNF-α), interleukin-6 (IL-6), interleukin-10( IL-10 ) and matrix metalloproteinase-9 (MMP-9) in serum of patients of acute organophosphorus pesticide poisoning (AOPP)with multiple organs dysfunction syndrome(MODS) and discussed the mechanism of MODS. Methods 42 patients with AOPP and MODS were enrolled in this study. TNF-α, IL-6, IL-10 and MMP-9 in their serum were detected with double antibody Sandwich-Enzyme linked with immunosorbent assay(DAS-ELISA) and compared with control group. The relations of TNF-α,IL-6,IL-10 and MMP-9 were analyzed with person correlation analysis. Results The contents of TNF-α, IL-6, IL-10 and MMP-9 elevated obviously in the serum of patients with AOPP,there was significant difference between patients with AOPP and normal control( P 〈 0.01 ). The level of MMP-9 had positive correlation with TNF-α, IL-6,IL-10. Conclusion AOPP may induce the disturbance of the inflammatory factor,and MMP-9 plays an important role in the development from SIRS to MODS in AOPP patients by regulating the inflammatory factors.
