摘要
目的:观察益气养阴、活血化瘀中药和针灸治疗糖尿病性视网膜病变的有效性、安全性,探讨益气养阴、活血化瘀中药和针灸治疗糖尿病视网膜病变的作用机制。方法:采用相同的入选和排除标准,纳入2型糖尿病非增殖期糖尿病视网膜病变病例130例,将患者随机分3组,中药组,中药加针灸组,多贝斯组,观察3个月。观察视力、视野、眼底(眼底镜、彩色眼底照相、荧光血管造影检查)、血液流变学及中医证候等疗效性指标。结果:益气养阴、活血化瘀中药及针刺眼区穴位(太阳、上睛明、承泣、球后、攒竹等穴),可以有效提高患者中心视力,改善视野的损害,促进视网膜点状,片状出血的吸收;益气养阴、活血化瘀中药联合针刺最为明显;单纯中药组优于多贝斯组,同时中药组与多贝斯组对糖尿病患者血液流变学异常均有改善作用,两者差异不显著。结论:眼底荧光血管造影证实活血化瘀中药及多贝斯均有改善或阻止DR患者视网膜损害状况如:微血管瘤,视网膜点片状出血,异常渗漏等。中药及针刺可以在积极有效控制血糖的前提下改善糖尿病视网膜病变患者受损的视功能、改善DR患者的病理变化或阻止其发展。
Objective ： Objective ： To study tile effect and safety and mecllanisms of traditional Chinese medicine with boosting qi and nourishing yin and quickenring the blood and transforming stasis and acupuncture in treating diabetic retinopathy. Methods; There were 130 type 2 diabetes mellitus patients with non-proliferative diabetic retinopathy enrolled in this study with the same inclusion criteria and exclusion criteria in the ciinic. They were randomly divided into thiee groups：a traditional Chinese medicine treated group, a traditional Chinese medicine with acupuncture treated group, and a calcium dobesilate treated group. The indices of visual acuity, visual field, fundus condition, hemorheology and TCM syndrome were evaluated in three months. Results ： Traditiooal Chinese medicine with boosting qi and nourishing yin and quickening the blood and transforming stasis effectively increased the central visual aeuity, improved the visual field, and improved absorption of retinal hemorrhage,and the traditional Chinese medicine with acupuncture treated group was most effective. The traditional Chinese medicine treated group was better than the calcium dobesilate treated group,mid they both ameliorating helnorrheologic indices. Conclusions： Traditional Chinese medicine with boosting qi and nourishing yin and quickening the blood and transforming stasis may improve and prevent the retinal damage. With the eontrol of blood sugar, traditional Chinese medicine and acupuncture may improve the visual function and improve or prevent the pathological damage in patients with diabetic retinopathy.
