摘要 目的:研究细胞周期蛋白D1(Cyclin D1)、P53蛋白和细胞周期蛋白依赖性激酶4(CDK4)在胰腺癌中的表达差异,阐明其与胰腺癌的相关性.方法:兰州大学第一医院和定西地区医院1997-07/2004-01手术切除胰腺癌患者48例,术后标本均经病理证实.采用免疫组化SP法检测Cyclin D1、P53和CDK4在胰腺癌患者不同临床指标下的表达水平.结果:胰腺癌不同性别、年龄分组中Cyclin D1、P53与CDK4的阳性表达率均无显著差异.在低分化、有淋巴结转移、临床分期Ⅲ,Ⅳ期的胰腺癌组织中Cyclin D1和CDK4的阳性表达率明显高于高分化、无淋巴结转移、临床分期Ⅰ,Ⅱ期的胰腺癌中的阳性表达率(CyclinD1:χ2=10.540,14.225,5.043,P<0.01或0.05;CDK4:χ2=7.619,4.006,5.581,P<0.05).有淋巴转移的胰腺癌组织中P53的阳性表达率明显高于无淋巴转移的组中的阳性表达率(χ2=6.146,P<0.05);而不同分化程度和临床分期的胰腺癌组织中P53的表达无明显的差异性.结论:Cyclin D1可以作为早期诊断胰腺癌的生物学指标,而P53和CDK4在胰腺癌的发生、发展过程中起重要的协同作用. AIM: To study the expression of Cyclin D1, P53 and Cyclin-dependent kinase (CDK4) in pancreatic carcinoma, and to explore the relationship of Cyclin D1, P53 and CDK4 with pancreatic carcinoma. METHODS: A total of 48 cases with pancreatic cancer were collected from the First Hospital of Lanzhou University and Dingxi district hospital from July 1997 to January 2004. Immunohistochemical staining (S-P method) was used to detect the expressions of Cyclin D1, P53 and CDK4 in 48 cases of pancreatic carcinoma. RESULTS: No significant difference between Cyclin D1, P53 and CDK4 expression was detected in different gender, age groups of pancreatic cancer. Positive expression rates of Cyclin D1 and CDK4 were significantly higher in the poorly differentiated, lymph node metastasis, clinical stage Ⅲ, Ⅳ phase groups than in well-differentiated, no lymph node metastasis, clinical stage Ⅰ, Ⅱ phase groups (Cyclin DI: Х^2 = 10.540, 14.225, 5.043, P 〈 0.01 or 0.05; CDK4:Х^2 = 7.619,4.006, 5.581, P 〈 0.05). The P53 positive expression rate was significantly higher in lymphatic metastasis group than the non-lymph node metastasis group (Х^2 = 6.146, P 〈 0.05). However, no significant difference in P53 expression was detected between different differentiation groups or between different clinical stage groups. CONCLUSION: As Cyclin D1 is over expressed in pancreatic carcinoma, it can be used as a biomarker for early diagnosis of pancreatic carcinoma. However, P53 and CDK4 play important synergistic roles in pathogenesis and development of pancreatic carcinoma.
