冠状动脉病变与左室结构关系分析

摘要 目的探讨冠心病患者冠状动脉病变范围及狭窄程度与左心室结构的关系。方法387例选择性冠状动脉造影,证明冠心病患者252例,按冠状动脉病变范围、狭窄程度分组,造影前行心脏超声检查,对比分析冠心病患者冠状动脉病变范围及狭窄程度与左室结构的关系。结果随着冠状动脉病变范围的扩大,狭窄程度的加重,主动脉内径、左室前后径、室间隔厚度、左室后壁厚度、左室质量指数逐渐增加(P<0.05或P<0.01)。结论冠状动脉病变对左室结构有一定影响,控制左室重构对冠心病患者具有重要意义。 Objective To investigate the relationship between the scope and degree of coronary artery stenosis and the structure of left ventricle. Methods Three hundred and eighty seven patients undergoing coronary arteriography(CAG)were chosen,of which, 252 patients diagnosed as coronary heart disease(CAD)by CAG were divided into different groups according to the extent of length and stenosis of the coronary artery. All patients underwent heart echocardiography before CAG. Based on the results of CAG and heart echocardiography, the correlation between the scope and degree of coronary artery stenosis and the structure of heart was analyzed. Results The inner diameter of aorta, anteroposterior diameter of left ventricle, thickness of interventricular septum and posterior left ventricular wall, left ventricle mass index(LVMI)increased significantly with the increasing severity of coronary artery lesion( P 〈 0.05 or P〈 0.01). Conclusion The coronary stenosis severity could influence the structure of left ventricle. To control the remodeling of left ventricular structure may do a great favor to CAD patients.
