摘要
目的探讨男性精神分裂症患者血清雌二醇、催乳素、睾酮和孕酮的水平及抗精神病药物对其影响。方法60例男性精神分裂症患者随机分为氯氮平组(30例)和利培酮组(30例),分别给予氯氮平和利培酮治疗4周,应用电化学发光免疫分析(ECLIA)技术分别于治疗前后测定血清雌二醇、催乳素、睾酮和孕酮水平,并与60名健康对照进行比较。结果治疗前患者组血清催乳素、孕酮高于对照组(P<0.05);治疗后患者组血清雌二醇、催乳素和孕酮高于对照组(P<0.05);与治疗前相比,治疗后氯氮平组血清催乳素显著升高(P<0.01),利培酮组血清雌二醇、催乳素、睾酮显著升高(P<0.01)。治疗后氯氮平组血清催乳素低于利培酮组(P<0.01),雌二醇、睾酮及孕酮的差异无统计学意义(P>0.05)。结论男性精神分裂症患者治疗前后均存在高催乳素状态,可能是精神分裂症的一种属性标志。氯氮平和利培酮均对男性精神分裂症患者性激素水平产生不同程度的影响。
Objective To explore the serum levels of estradiol, prolaetin, testosterone and progesterone in male patients with schizophrenia and their effects of antipsyehotics. Methods All 60 male patients with schizophrenia were divided into the clozapine group （n=30） and risperidane group （n=30） randomly, in which clozapine and risperidone was alone prescribed for 4 weeks. The serum levels of estradiol, prolactin, testosterone and progesterone of all the patients were measured with electro-chemiluminescence immune assay （ECLIA） instrument at baseline and after 4 weeks treatment respectively. The 60 healthy persons matched with study groups were recruited as the control group. Results The levels of serum prolaetin and progesterone in study group were higher than those of control group （P〈0.05） before treatment and the serum levels of estradiol, prolactin and progesterone in study group were higher than those of control group after treatment （P〈0.05）. After treatment, the levels of serum prolactin in patients treated with clozapine were higher than those before treatment （P〈0.05） while there were no significant differences in estradiol, testosterone and progesterone （P〉0.05）. The serum levels of estradiol, prolactin and testosterone in patients treated with risperidone were higher than those before treatment （P〈0.05） but there was no significant difference in progesterone （P〉0.05）. The serum levels of prolactin in clozapine group were lower than those of risperidone group （P〈0.01） and there were no significant differences in estradiol, testosterone and progesterone after treatment （P〉0.05）. Conclusion Hyperprolactinemia in male patients with schizophrenia before and after treatment may be a strait-marker of schizophrenia. Clozapine and risperidone have different effects on serum levels of sex hormones in male patients with schizophrenia.
