膨肺吸痰法在冠脉搭桥机械通气期间的研究 被引量:4

摘要 目的探讨膨肺吸痰在冠脉搭桥机械通气期间预防并发症的意义。方法72例冠脉搭桥术后患者随机分为两组,观察组40例,对照组32例,观察组应用膨肺吸痰法,对照组应用常规吸痰法,观察两组患者吸痰后血气分析及肺功能改变情况以及比较两组机械通气时间、肺不张发生率、肺部感染发生率。结果吸痰后观察组与对照组动脉血气分析及肺功能比较,差异有显著性(P<0.05);观察组机械通气时间为12.30±2.21天,对照组为17.01±2.39天,差异有显著性(P<0.05)。结论膨肺吸痰可有效保持呼吸道通畅,减少负压吸引可能引起的肺不张及低氧血症,并能缩短机械通气时间,降低肺不张、肺部感染发生率。促进心脏手术后病人早日康复。 Objective To explore the effect of lung inflation and sputum aspiration on preventing complications during mechanical ventilation in patients undergoing coronary bypass. Methods 72 patients with mechanical ventilation after open hearts surgery were randomly divided into 2 groups. The observation group was treated with lung dilated suction, while the control group was treated with routine suetioning. Then two groups' blood gas analysis and the respiratory function index were observed and the duration of mechanical ventilation.the incidence of pulmonary ateleetasis and pulmonary infection were compared before and after suetioning. Results Blood gas analysis and the respiratory function index in observation group have statistic meaning (P〈0.05)before and after suetioning. The duration of mechanical ventilation have statistic meaning (P〈0.05) in two groups. Conclusion Lung inflation and sputum aspiration play an important role in keeping respiratory tract unobstruet, lessening the incidence of ateleetasis and hypoxemia caused by vacuum suction, reducing cardiac load and promoting the recovery.
