摘要
目的:通过与DSA比较,评价增强磁共振血管造影(CE-MRA)在颅内动脉瘤诊断中的临床应用价值。材料和方法:对85例临床怀疑有颅内动脉瘤的病人行CE-MRA检查,随后1周内行DSA造影。CE-MRA采用透视触发技术和三维小角度激发快速梯度回波序列,行增强前、增强后动脉期和静脉期3次扫描,图像减影后行三维重建,重建图像包括MIP、VR和MPR,比较CE-MRA与常规DSA对颅内动脉瘤的显示。结果:发现65例病人共69个动脉瘤,CE-MRA诊断动脉瘤的敏感性、特异性和准确性分别为98.5%、85%、95.5%,CE-MRA上能够准确测量动脉瘤大小与瘤颈宽度,与DSA测值无统计学差异。对于瘤内血栓的显示,CE- MRA更优于DSA。结论:无创CE-MRA能有效的诊断颅内动脉瘤,所提供的三维信息对指导临床治疗方案的选择具有极大帮助。
Purpose: To evaluated the clinical value of contrast - enhanced MR angiography in the detection for intracranial aneurysm by comparing with DSA. Materials and Methods : CE - MRA was performed in 85 patients suspected with intracranial aneurysms.M1 patients received DSA within the same week. CE - MRA used fluoroscopic triggering technique and three - dimensional fast low angle shot (3D FLASH) sequence were used for CE - MRA. Scan repeated three times (pre - contrast, arterial phase and venous phase), the source images were subtracted and transferred to computer workstation using 3D reconstructed, reconstruction images include MIP, VR and MPR. CE - MRA images and conventional DSA images were compared for demonstration of the aneurysms. Results: There were 65 cases with 69 intracranial aneurysms. The sensitivity, specificity and accuracy of CE - MRA were 98.5% , 85% and 95.5%, respectively. The sizes and necks of aneurysm can be measured on CE - MRA images accurately, compared with conventional DSA , there was no statistical difference. CE - MRA was superior to DSA in displaying the thrombus inside aneurysms. Conclusion: CE - MRA is a fast, noninvasive and efficient technique for diagnosing intracranial aneurysms. Its 3D information is helpful for treatment planning.
