踝关节骨挫伤的MRI诊断及鉴别诊断 被引量:4

摘要 目的:探讨踝关节骨挫伤的MRI诊断及鉴别诊断。材料和方法:总结分析11例踝关节外伤性骨挫伤的MR表现,总结其特征。结果:11例病人14个踝关节,17处病灶:胫骨远端7处,距骨滑车5处,胫骨干1处,胫骨内踝2处,腓骨外踝2处。MR形态特点:小碎片状2处;半月形5处;星芒状9处;不规则地图形1处。MR信号特点:9处病灶T_1WI为等信号,8处为低信号;11处病灶T_2WI表现为稍高信号,另6处病灶为等信号中可见星点状稍低信号;全部17处病灶去脂T_2WI均表现高信号。结论:踝关节骨挫伤MR表现特点明确,广泛应用可提高早期诊断率。 Purpose: To explore the MRI diagnosis and differential diagnosis of the ankle bone contusion. Materials and Methods: The MRI appearances of 11 cases of bone contusion in the ankles were retrospectively analyzed and the MRI characteristics of them were summarized. Results: Total of 14 ankle joints of 11 patients , 17 bone contusions were found, of which 7 were located in the distal tibia, 5 in the trochlea tali, 1 in the tibia] diaphysis, 2 in the medial malleolus of tibial and 2 in the external malleolus of fibula. MR imaging showed shape features of patchy in 2 of them, half-moon in 5, starlight in 9, irregular geographic in 1. MR showed signal features of isointense signal in T1WI of 9 foci, hypointense signal in T1WI of 8 foci, hyperintense signal in T2WI of 11 foci, isointense signal with a little starry hypointense signal in T2WI of 6 foci ,hyperintense signal in fat suppression T2WI of all foci. Conclusion: The manifestation of bone contusion of the ankle in MR is characteristic and MR examination will in- crease the diagnostic accuracy of this disease.
