经三维CT定位的周围支射频温控热凝治疗三叉神经痛 被引量:7

Trigeminal Peripheral Branches Radiofrequency Thermocoagulation under the Guide of 3D Computerized Tomography for the Treatment of Idiopathic Trigeminal Neuralgia

摘要 目的:探讨三维CT重建定位下经周围支射频温控热凝术治疗三叉神经痛的疗效。方法:将160例原发性三叉神经痛患者随机分为2组,每组80例.分别在三维CT重建及非重建条件下.经三叉神经周围支及半月节行射频热凝治疗,统计分析其术后即刻疗效及复发率。结果:术后即刻疗效比较,重建引导组有效率93.75%,非重建引导组有效率92.50%,无显著性差异(P>0.05)。经1~4.5年的随访,重建引导组总复发率31.25%,非重建引导组复发率47.50%,重建引导组复发率显著低于非重建引导组(P<0.05):涉及三叉神经第一支射频治疗22例.复发3例。结论:三维CT重建定位指导下,经三叉神经周围支行射频温控热凝术操作方便、安全、适应证广,止痛效果确切,远期疗效优于直接穿刺射频。 Objective: To investigate the therapeutic effect of trigeminal peripheral branches in radiofrequency thermocoagulation under the guide of 3D computerized tomography for the treatment of idiopathic trigeminal neuralgia. Methods: 160 patients with idiopathic trigeminal neuralgia were divided into 2 groups randomly and equally. All patients in group 1 were underwent radiofrequency thermocoagulation of trigeminal peripheral branches or semilunar ganglion and guided by 3D computerized tomography .Patients in group 2 were underwent trigeminal peripheral branches or semilunar ganglion radiofrequency thermocoagulation. The immediately therapeutic effect and the recurrence rate of 2 groups were statistically ananlyzed. Results: The immediately effective rate of guided group was 93.75%, while it was 92.50% in group 2. There was no significant differences between them (P〉0.05). After 4.5 years follow up, the recurrence rate of guided group was 31.2% and it was 47.50% in without guide group, and the former is smaller than the later significantly (P〈0.05). Conclusion: Trigeminal peripheral branches radiofrequency thermocoagulation under the guide of 3D computerized tomography for the treatment of idiopathic trigeminal neuralgia is convenient and safe, therapeutic effect is certain .
