摘要
目的:探讨芳香化酶P450及雌激素受体(ER)、孕激素受体(PR)在子宫内膜增生症及内膜癌组织中的表达,分析与子宫内膜癌发生机制、临床分期、细胞分化的相关性。方法:应用SP免疫组化法对子宫内膜单纯性增生52例、复杂性增生38例、不典型增生36例、内膜样腺癌58例患者的芳香化酶P450表达及ER、PR进行检测,并以42例正常增生期子宫内膜作为对照。结果:正常增生期子宫内膜无芳香化酶P450的表达,而在单纯性增生、复杂性增生、不典型增生的子宫内膜组织、内膜样腺癌组织中芳香化酶P450的表达均为阳性,表达率分别为40.4%、47.4%、69.4%和72.4%,与正常增生期子宫内膜相比差异有统计学意义;不典型增生与内膜样癌相比差异无统计学意义。正常增生期子宫内膜、子宫内膜单纯性增生、复杂性增生、不典型增生、内膜样腺癌的ER阳性表达率分别为59.5%、61.5%、57.5%、61.1%和60.3%,PR阳性表达率分别为73.8%、73.1%、73.7%、72.2%和74.1%,各组间无统计学差异,且与芳香化酶P450的表达无相关性。芳香化酶P450在子宫内膜癌不同临床分期、不同分化级别中的表达差异无统计学意义。结论:芳香化酶P450的不同表达与子宫内膜增生、癌变的发生发展有相关性,是子宫内膜发生不典型增生和癌变的促进因素之一。
Objective : To study the expression of aromatasecy P450 and receptor of estrogen and progesterone in the tissue of endometrial hyperplasia and endometrial cancinoma, analysis the correlation of the endometrial carcinoma pathogenesis, clinical stage and cell differentiation. Methods : The immunohistochemiscal S-P method techniques, were performed to study the expression of aromatasecy P450, estrogen receptor(ER) and progesterone receptor(PR) in the tissue of endometrium from 226 cases, including 42 cases of normal endometrium, 52 cases of simple endometrial hyperplasia, 38 cases of complex endometrial hyperplasia, 36 cases of endometrial atypical hyperplasia and 58 cases of endometrial carcinoma. Results: There was no the positive expression in the normal endometrial tissues. But the positive expressions of aromatasecy P450 were found in the tissue of endometrial hyperplasia (simple endometrial hyperplasia, complex endometrial , atypical hyperplasia )and endometrial carcinoma . The rates of the positive expression were 40.4%, 47.4%, 69.4%, 72.4% respectively. There were significant relationships among fifth groups( P 〈0.01 ). The rates of the positive expressions of ER in the tissue of normal endometrial, endometrial hyperplasia (simple endometrial hyperplasia, complex endometrial , atypical hyperplasia )and endometrial carcinoma were 59.5% ,61.5% ,57.5% ,61.1% ,60.3% ,. The rates of the PR were 73.8%, 73.1%, 73.7%, 72.2%, 74.1%. There were no relationship between the rates of the positive expressions of ER and PR and degree of the histological atypia. There were no relationship between the expressions rates of aromatasecy P450 and the expressions of ER and PR in endometrial hyperplasia and endometrial carcinoma. Conclusion : Aromatasecy P450 may be a factor that induces endometrial hyperplasia and carcinomatous changes. And it may become a biological marker in early carcinomatous changes in endometrial tissues.
