摘要
目的评价应用心肌作功指数(Tei指数)方法优化双腔起搏器房室延迟参数后,Tei指数与主动脉瓣、二尖瓣血流频谱时间流速积分的相关性。方法24例因Ⅲ°房室传导阻滞接受永久起搏器治疗的患者,于植入术后行常规超声及脉冲多普勒检查,应用最小Tei指数方法选择最佳房室延迟时间(优化组)。结果与非优化组相比,优化组的主动脉瓣、二尖瓣血流频谱时间流速积分(TVI-AV、TVI-MV)及左室充盈时间(FT)均较高(无统计学差异),优化组的Tei指数与TVI-MV、TVI- AV及FT的相关系数分别为-0.815、-0.744(P<0.01)和-0.641(P<0.05)。结论房室延迟参数优化中应用Tei指数方法与左室做功有较好的相关性。
Objectives To evaluate the correlation between time-velocity integral of aortic and mitral valve and myocardial performance index (Tei-index) on atrio-ventricular (A-V) delay optimization after implanting the dual chamber pacemaker. Methods Twenty-four patients with complete atrioventricular block and received permanent pacemakers were examined by two-dimensional and pulse Doppler echocardiography at different A-V delay. Results The best A-V delay was determined by Tei-index method, Time-velocity integral of aortic and mitral valve were higher in the best A-V delay, among which the correlation was higher between Tei index and time-velocity integral of aortic and mitral valve(r = -0. 815, -0. 744, P 〈 0.01 ). Conclusion A good correlation was found between time-velocity integral of aortic and mitral valve and myocardial performance index on A-V delay optimization.
