中老年慢性肾脏病患者双能X线骨密度测定结果分析 被引量:6

摘要 目的:观察中老年慢性肾脏病(CKD)患者骨密度变化,了解骨质疏松患病情况。方法:以305名中老年(年龄≥50岁)慢性肾脏病3~5期且尚未透析治疗患者为观察对象,102名非慢性肾脏病中老年志愿者为对照组。采用双能X线骨密度(DXA)仪测定腰椎正位(L1~4)和股骨等部位的骨密度(BMD),观察CKD患者骨质疏松患病情况。按性别或CKD分期进行再分组,进一步观察不同性别或不同CKD分期条件下患者骨质疏松患病情况。结果:中老年慢性肾脏病患者总骨质疏松患病率为37.8%,男性组患病率为28.2%,女性组患病率49.2%,与对照组相比均有明显升高(P〈0.01或P〈0.05)。按CKD分期观察,随慢性肾衰竭程度加重,骨质疏松患病率逐渐增加,其中CKD5期患者与CKD3、CKD4期患者比较其骨质疏松患病率增加更加明显(P〈0.01或P〈0.05)。结论:中老年慢性肾脏病患者具有较高的骨质疏松患病率;随慢性肾衰竭程度加重,骨质疏松患病率有逐渐增加趋势;慢性肾衰竭是否为骨质疏松症的危险因素尚需探讨。 Objective:To analysis the bone mineral density (BMD) and the prevalence rate of osteoporosis in middleaged and elderly patients with chronic kidney disease. Methods:305 middle- aged and elderly patients with chronic kidney disease (CKD 3- 4 stage, aged over 50 years) were included and 102 peoples without CKD were set as control group. The bone mass density at the lumbar spine (L1-4 ) and femur was detected by dual energy. X- ray absorptiometry (DXA). The prevalence rate of osteoporosis was analysed in total CKD patients and subgroups according to the sex and CKD stages. Results:The total prevalence rate of osteoporosis in CKD patients was 37.8 % ,of whom the rate of males was 28.2 % and that of females 49.2 %. There was very significant difference in prevalence rates between the CKD patient group and the control group (P 〈 0.01 or P 〈 0.05). The prevalence rate of osteoporosis was increased with the decline of the renal function, especially in patients with severe CKD. There was much higher prevalence rate of osteoporosis in CKD 5 stage patients than that in CKD 3 or 4 stage patients( P〈0.01 or P〈0.05). Conclusion.There was higher prevalence rate of osteoporosis in middle - aged and elderly patients with chronic kidney disease. The prevalence rate of osteoporosis was increased with the decline of the renal function. Whether CKD is a risk factor for osteoporosis needs more research.
