摘要
目的探讨培多普利对酒精性心肌病(ACM)心衰患者左室重构的影响。方法将52例酒精性心肌病合并心衰患者随机分为2组,均给于强心、利尿、营养心肌、β-受体拮抗剂等常规治疗。在此基础上,治疗组给培多普利4～8mg,1次/d。对照组给卡托普利25～50mg,3次/d。均从小剂量用起,治疗时间3个月。治疗前后分别测定2组患者的左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)和左室射血分数(LVEF)。结果治疗前后比较,2组的LVEF值均明显提高(P<0.05),LVEDD和LVESD显著缩小(P<0.05)。培多普利组在3个月后LVEDD、LVEF比卡托普利组变化明显(P<0.05)。结论培多普利较一般ACEI更有效地改善ACM合并心衰患者的心功能,抑制左室重构。
Objective To study the effect of Perindopril on left ventricular remolding in patients with alcoholic myocardiopathy （ACM） complicating heart failure. Methods 52 patients with ACM complicating heart failure were randomly divided into thera- py group（ Perindopri1,4-8 mg, once daily, n = 26 ） and control group（ Captopril 25-50 mg, three times daily, n = 26）. Both groups were given three-month-routine therapy including Digoxin, diuretic, nutrition of cardiac muscle and B-receptor antagonist. Left ventricular end-diastolic dimension（LVEDD） ,left ventricular end-systolic dimension（LVESD） and left ventricular ejection frac- tion（LVEF） were detected respectively before and after the treatment. Results Compared with pretherapy respectively, LVEF was improved whereas LVEDD and LVESD decreased significantly after three-month treatment in both groups（P 〈 0. 05 ）. There was obviously difference in LVEDD and LVEF between the treatment group and the control group （ P 〈 O. 05 ）. Conclusions Compared with Captopril, Perindopril can more effectively improve heart function and inhibit left ventricular remolding of patients with ACM complicating heart failure.
