摘要
目的探讨不同方法治疗老年人股骨转子间骨折的疗效,选择最佳治疗途径。方法回顾分析52例老年转子间骨折患者分别经保守治疗,手术治疗(DHS、DCS、PFN、Gamma钉、Richards、股骨近端解剖钢板)的治疗效果。结果全部患者得以随访,随访时间4～36个月,除1例患者死于围手术期并发症,其余患者8～12周全部骨性愈合,功能恢复良好,优良率非手术组70%,DHS组90%,其它内固定组86.4%。结论老年人股骨转子间骨折各种方法治疗各有其特点,DHS内固定具有固定牢固、方法简单、早期下床活动等优点。
Objective To research the clinical effect of different treatments on senile intertrochanteric fracture and find out the best one. Methods 52 elderly patients with intertrochanteric fracture which were treated by conservative or surgical（ DHS, DCS, PFN, Gamma pin, Richards, proximal femoral anatomic plate） method were analyzed retrospectively. Results All of the cases were fallowed up within 4-36 months. 51 bone union and well functional recovery was achieved between 8 and 12 weeks, except 1 died of perioperative complications. The excellent and good rate of conservative method was 70% ,while DHS was 90% ,and other internal fixation methods was 86.4%. Conclusions Kinds of treatments for senile intertrochanteric fracture have different fea- ture, DHS internal fixation has the advantages of firm fixation, simple operation and early ambulation etc.
