摘要
目的探讨彩色多普勒超声(CDFI)对乳腺良、恶性肿瘤的诊断价值。方法对240例乳腺肿瘤患者的彩色多普勒超声结果进行回顾性分析,并与手术病理对照,总结良、恶性肿瘤的声像特点。结果240例乳腺肿瘤均行手术治疗,超声诊断乳腺癌62例,乳腺纤维腺瘤168例,导管内乳头状瘤6例,脂肪瘤4例,病理诊断乳腺癌61例,乳腺纤维腺瘤166例,导管内乳头状瘤6例,脂肪瘤4例,增生结节2例,超声诊断乳腺癌的符合率为98.4%,乳腺纤维腺瘤准确率98.8%,两种肿瘤的回声均以低回声为主,而乳腺恶性肿瘤形态不规则,内部回声不均质,CDFI血流丰富,阻力指数(RI)呈高阻血流,良性肿瘤无血信号或少量及点状血流信号,RI呈低阻血流。结论彩色多普勒超声对乳腺良、恶性肿瘤的诊断准确率高,对临床手术方式的选择有较高参考价值。
Objective To explore the diagnostic value of Color Doppler Flow Imaging （ CDFI ） for the breast tumor. Methods The CDFI of 240 cases breast tumor was analyzed retrospectively,as well as compared to their operation pathological results. Results In all 240 cases,according to the CDFl,there were 62 cases breast carcinoma, 168 cases breast fibroadenoma,6 cases intraductal papilloma,4 cases lipoma, while according In the operation pathological results, there were 61 cases breast carcinoma, 166 cases breast fibroadenoma ,6 cases intraductal papilloma,4 cases lipoma ,2 cases hyperplastic nodule. The coincidence rate of CDFI breast malignant tumor diagnosis was 98.4% , while to the breast fibroadenoma, was 98.8%. Although whole breast tumor showed low echo level in color Doppler, but to the malignant tumor, which had irregular outline,inhomogen internal echo,abundance blood flow and high value in Resistance Index（RI） ,while to the benign tumor, which showed no or little blood/low and low value in RI. Conclusion The color Doppler has great value in the breast malignant/ benign diagnosis ,which will be helpful for the surgery treatment.
