摘要
检验血流频谱分析在鉴别乳腺原发性肿块良恶性方面的价值并确立最佳的数值标准.方法:使用彩色多普勒血流显像(CDFI)和能量多普勒显像(PDI)分别引导脉冲多普勒(PWD)频谱分析,检查了104例乳腺原发性包块(良性55例63个病灶,恶性49例64个病灶),测量指标包括肿块周边与内部血流的收缩期峰速(PSV)、搏动指数(PI)及阻力指数(RI),井分别绘制受试者工作曲线确定各自的最佳阈值.结果:以CDFI状态取频谱,乳癌病灶周边及内部血流的平均PSV,PI和RI值均明显高于良性病变.而以PDI引导频谱分析,良恶性之间的差别较小.肿瘤周边血流PSV35.0cm/s,PI1.4和RI0.72是鉴别乳腺肿瘤良恶性的最佳阈值.结论:以彩色多普勒血流显像引导肿瘤血流频谱分析,对乳腺肿瘤良恶性的鉴别具有较高价值.
To evaluate the value of blood flow frequency spectral analysis in the differentiation of primary breast neoplasm and establish the thresholds for this purpose. Methods: One hundred and four primary breast lumps, including 55 benign changes and 49 malignant tumors, were examined with pulsed wave Doppler (PWD) directed by color Doppler flow imaging (CDFI) and power Doppler imaging (PDI). Results: Peak systolic velocity(PSV), pulsatility index (PI) and resistive index(RI) of intratumoral and peripheral flow of malignant tumors were obviously higher than that of benign ones. It was manifested on ROC that PSV 35. 0 cm/s, PI 1. 4 and RI 0. 72 were the best thresholds for differentiating breast neoplasms. Conclusion: Blood flow frequency spectral analysis may play an important role in the differentiation of breast neoplasm.
