摘要
目的 通过检测674例强直性脊柱炎（AS）患者HLA-B27抗原阳性频率和表达强度，了解四川地区AS与HLA-B27抗原的相关性，并初步探讨其临床意义。方法 采用流式细胞术检测AS患者T淋巴细胞膜上的HLA-B27抗原和其平均荧光强度（mean fluorescence intensity，MH）。研究样本来源于两家医院临床诊断为AS的患者。结果 ①674例AS患者以16～50岁青壮年为主，占91．1％；男女比例为3．0：1；男女患者就诊年龄分别为（30．4±10．7）岁和（35．4±12．1）岁（P〈0．01）；②674例AS患者的HLA-B27抗原阳性率97．9％；男女患者HLA．B27抗原阳性率分别为98．4％和96．5％（P〉0．05）；男女阳性患者的MFI有明显差异（P〈0．01）；③骶髂关节炎在I级和Ⅱ级以上的患者HLA-B27抗原阳性率和荧光强度明显高于O级者（P〈0．01）；④幼年AS患者男女比例为6．7：1（20：3）；HLA-B27抗原阳性率100％；其HLA-B27抗原MFI明显低于成年AS患者（P〈0．01）。结论 四川地区AS患者与HLA．B27抗原有着很强的相关性。采用流式细胞技术检测临床疑似AS患者T淋巴细胞HLA-B27表达强度有助于诊断和鉴别诊断AS，也有利于临床对病情严重程度和进展的判断。
Objective To investigate the relationships between Ankylosing Spondylitis and HLA-B27 antigen in siduan province by deteeting the positive frequency and expression of HLA-B27 antigen in 674 diagnosed AS patients, and evaluate its clinical importance. Methods The positive frequency and the mean fluorescence intensity（ MFI）of HLA-B27 antigen on the thymus lymphocyte membrane were detected by flow cytometer. All samples were come from two local hospitals.Results ①Among the 674 AS patients,the majority patients（91.1% ）were aged from 16 to 50 years old,and the ratio between male and female was 3.0： 1 ;The diagnostic age of male and female were （30.4±10.7） and （35.4 ± 12.1 ）respectively（ P 〈 0.01 ）. ②Among the all patients, the positive frequency of antigen HLA-B27 was 97.9%, and the male and female patients＇ positive frequencies of antigen HLA-B27 were 98.4% and 96.5% respectively （ P 〉 0.05 ） ,and the MFI of the positive male and female patients were different significantly（ P 〈 0.01 ）. ③Both of the positive frequency and MFI of antigen HLA-B27 of patients with stage I or Ⅱ sarocoxitis were higher than those with stage O（ P 〈 0.01 ） .④The positive frequency of antigen HLA-B27 of juvenile AS （JAS）patients （6 - 15years old）was 100% , and the ratio between boys and girls was 6.7：1 （20：3）. The MF I of antigen HLA-B27 in JAS patients were lower than those in adult patients （ P 〈 0.01 ）. Conclusions The Chinese Han patients with AS in Sichuan Province are grongly associated with antigen HLA-B27. Detection of antigen HLA-B27 expression intensity in suspected AS individuals with FCM in helpful to confirm diagnosis of AS,and useful to estimate the state of illness.
