甲状腺功能亢进患者胰岛素抵抗和胰岛素分泌功能的变化 被引量:10

摘要 目的探讨甲亢患者胰岛素抵抗与胰岛素分泌功能的变化及其影响因素。方法采用Homa Model模型计算胰岛B细胞分泌功能指数(HOMA-IS)、胰岛素抵抗指数(HOMA-IR)和胰岛素敏感指数(ISI)作为反映胰岛B细胞分泌功能和胰岛素抵抗程度的指标,对甲亢患者胰岛B细胞分泌功能和胰岛素抵抗程度进行测定,同时将其与患者的年龄、体质指数和甲状腺激素水平等作多因素回归分析。结果甲亢患者的HOMA-IR明显增高,而ISI及HOMA-IS明显下降;FT3是影响ISI和HOMA-IR最重要的因素(偏相关系数分别为-0.332和0.332,P<0.001)。结论甲亢患者的糖代谢紊乱及胰岛素抵抗现象与甲状腺激素水平紧密相关。
