尿毒症维持性血液透析患者血浆可溶性CD_(40)配体的变化及临床意义 被引量:4

Changes of Plasma Soluble CD40L in Uremic Patients undergoing Maintenance Hemodialysis and Its Clinical Significance

摘要 目的观察尿毒症维持性血液透析(MHD)患者血浆可溶性CD40配体(sCD40L)的变化以及与血管性假性血友病因子(vWF)和组织因子(TF)之间的关系,进一步探讨MHD患者动脉粥样硬化的发病机制。方法应用酶联免疫吸附试验(ELISA)测定48例MHD患者(MHD组)和22例健康志愿者(对照组)血浆sCD40L、vWF和TF水平,并对检测结果进行统计学分析。结果MHD组血浆sCD40L、vWF和TF的水平与对照组比较差异有统计学意义(P<0.05),MHD伴冠心病组sCD40L、vWF和TF水平与不伴冠心病组比较差异亦有统计学意义(P<0.05);直线相关分析表明sCD40L水平与vWF、TF水平均呈正相关(P<0.01);Logistic回归分析表明:sCD40L、vWF和TF与冠心病的发生有关(P<0.05)。结论MHD患者尤其是伴有冠心病患者sCD40L、vWF和TF水平升高,可能参与动脉粥样硬化的形成;sCDL可能通过上调vWF和TF的表达而促进冠心病的发生、发展。 Objective To investigate the changes of plasma soluble CD40ligand (sCD40L) and it's relationship to vascular pseudo - van Willebrand factors (vWF) and tissue factors (TF), to probe further into the pathogenesis of atherosclerosis in uremic patients undergoing maintenance hemodialysis (MHD). Methods Enzyme - linked Immunosorbent Assay was used to measure the plasma sCD40 L, vWF and TF in maintenance hemodialysis patients ( MHD group, n = 48 ) and healthy volunteers ( control group, n = 22). Results ( 1 ) The levels of sCD40 L, vWF and TF were significantly higher in MHD group than those in control ( P 〈 0. 01). The levels of sCD40L, vWF and TF were significantly higher in MHD patients with coronary heart disease (CHD) than those without CHD ( P 〈 0. 05 ) ; (2) Linear correlation analysis showed that sCD40L level was positively relatedto vWF (P 〈0.01), and TF (P 〈0.01); (3) Logistic regression analysis showed that sCD40 L, vWF and TF were related to the occurrence of CHD ( P〈 0.05 ). Conclusion The elevated levels of sCD40 L, vWF and TF in MHD patients, particularly in those with CHD, may partake in the formation of atherosclerosis, and the positive correlation of sCD40 L with vWF and TF may indicate the possible mechanism in which sCD40L facilitates CHD occurrence by up -regulating vWF and TF expression.
