血清MUC1、MUC2及MUC5AC表达与胃癌的关系

摘要 目的:检测胃癌患者血清中MUC1、MUC2及MUC5AC含量,探讨与胃癌的关系。方法:制备MUC1、MUC2及MUC5AC黏蛋白芯片,免疫荧光检测,采用CEA作为指示指标,对30例胃癌患者及30例健康人进行黏蛋白血清水平的检测。结果:胃癌组三种黏蛋白血清水平增高,两组间差异显著(P<0.01)。MUC1的阳性表达与胃癌TNM分期相关(P<0.01),MUC2、MUC5AC的阳性表达与TNM分期无关(P>0.05)。三种黏蛋白(MUC1、MUC2、MUC5AC)的表达随胃癌分化程度降低有增高的趋势。各单一黏蛋白以及黏蛋白两两之间和联合诊用于胃癌诊断的敏感性及特异性均较高,其中单一黏蛋白中,MUC1的的敏感性和特异性最好,分别达到81.5%、75.8%,联合诊断(三种黏蛋白中至少两种阳性)的敏感性和特异性达到96.0%、82.9%。结论:检测血清黏蛋白MUC1、MUC2及MUC5AC水平,可以提高胃癌诊断的敏感性及特异性,为胃癌的早期诊断提供新的思路。 Objective: To study the relationship between the gastric cancer and the expression of MUC1, MUC2 and MUC5AC in serum. Methods: Mucins' protein chip was established and applied to measure the serum levels of MUC1, MUC2 and MUC5AC in 30 gastric cancer patients and 30 healthy adults as control, using the method of immuno-fluorescence. Results: The serum mucins and CEA levels of gastric cancer patients were significantly higher than that of the healthy ones (P 〈0.01 ). The positive expression of the MUClwas related with the gastric cancer's TNM stage( P 〈0.01 ), The positive expression of the MUC2 and MUC5AC had not relationship with the gastric cancer's stage( P 〉 0.05 ). The positive expression of the mucins was related with the gastric cancer's differentiation. The sensibility and specificity were both higher in mucins when used to diagnose. The sensibility and specificity of MUC1 were 81.5%. 75.8% respectively. The sensibility and specificity of combined mucins (at least 2 m 3 mucins were positive) were 96.0%. 82.9%. Conclusion: To measure the serum levels of MUC1, MUC2 and MUC5AC could promote the sensibility and specificity in the diagnosis of gastric cancer and provide a new approach to early diagnose gastric cancer.
