摘要
观察15例老年呼吸系统感染患者体内奈替米星(Net)的药物动力学.用高效液相色谱-间接光度检测(HPLC-IPD)法,烟酰胺为检测剂,庚烷磺酸钠为反离子,测定单剂量和多剂量im及iv gtt 5mg/kg Net后的血清药物浓度,并计算主要药物动力学参数;同时测定了痰液药物浓度.结果:Net在体内消除半衰期为3.5h左右,单剂量im达峰时间0.77h,峰浓度为14.33mg/L.多剂量给药后体内轻微积蓄,痰药浓度与血药浓度无相关性.提示老年呼吸系统感染患者对Net消除较慢,生物利用度增高.
The pharmacokinetics of netilmicin sulfate was investigated in 15 old patients contracted respiratory system infection. The concentration of netilmicin sulfate was determined by a high performance liquid chromatography-indirect photometric determination (HPLC-IPD) method. The chromatographic system consisted of Spherisorb C 18 column and mobile phase of a methyl alcohol-buffer of acetic acid (pH4. 5) (75: 25), which contained 0. 4mmol/L of nicotinamide and 0. 3mmol/L of sodium seventhalkyl -sulphonate. The wave-length of detection was 262nm, After intramuscular injection and intravenous drip administration 5mg/kg of netilmicin sulfate ,the data obtained were fitted with a 3P87 program on computer. The dispostion was conformed to a two-compartment model with T1/2β=3. 65h, Tmax = 0. 77h,Cmax = 14. 33mg/L at intramuscular injection .There was a slight accumulation at repeated dose. The concentration of netilmicin sulfate in phlegm was also determined and there was no correlation with serum concentration.
