摘要
目的探讨ATPⅢ(2001)、CDS(2004)、IDF(2005)等三种代谢综合征定义在中国冠心病人群中应用价值。方法于2005年6月1日至2005年9月30日期间在北京、上海、广州等7个城市共52家三级甲等医院心内科所有符合冠心病诊断纳入条件的住院患者连续入选为研究对象,共收集有效病例3 513例。分别应用ATPⅢ、CDS以及IDF的工作定义诊断MS,比较各定义下的MS的患病率以及通过运用ROC曲线来考查这三种工作定义两两间比较的一致性情况。结果ATPⅢ、CDS以及IDF工作定义下冠心病患者MS患病率分别为40.2%、52.8%和53.5%。ROC曲线特征:CDS为参照下,ATPⅢ和IDF的灵敏度、特异度、曲线下面积分别为0.62、0.84、0.73和0.73、0.68、0.70。IDF为参照下,ATPⅢ和CDS的灵敏度、特异度、曲线下面积分别为0.59、0.82、0.70和0.72、0.69、0.70。ATPⅢ为参照下,IDF和CDS的灵敏度、特异度、曲线下面积分别为0.79、0.53、0.71和0.81、0.66、0.74。结论ATPⅢ、CDS、IDF工作定义侧重点不一,IDF定义下的MS患病率最高。
Objective To discuss the application values of the metabolic syndrome (MS) in patients hospitalized with coronary diseases (CHD) in China according to the Third Report of the National Cholesterol Educational Program Expert Panel in 2001 ( NCEP-ATP Ⅲ ), Chinese Diabetes Society ( CDS ) in 2004 and International Diabetes Federation (IDF) in 2005. Methods From June 1, 2005 to Sept. 30, 2005, fifty two hospitals in 7 cities of China participated in the survey, in which all the patients with CHD in cardiology department of each hospital were recruited consecutively. As a result, a total of 3 513 patients with CHD were included in this study. MS were diagnosed by these three definitions respectively. Then the MS prevalence were compared and the agreements of these three criteria were evaluated by the Receiver Operating Characteristic (ROC)curves. Results The MS prevalence were 40.2% ,52.8% ,53.5% among the subjects according to ATPⅢ ,CDS and IDF respectively. In the light of the criterion of CDS, the sensitivity ,specificity and the acreage under the curves(AUC) between ATPⅢ and IDF were 0. 62,0. 84,0. 73 and 0. 73,0. 68,0. 70 respectively. And in the light of the criterion of IDF, the three parameters between ATP Ⅲ and CDS were 0. 59,0. 82,0. 70 and 0.72, 0.69,0.70 respectively. And in the light of the criterion of ATP Ⅲ, the three parameters between IDF and CDS were 0. 79,0. 53,0. 71 and 0. 81,0. 66,0. 74 respectively. Conclusion Different aspects were emphasized among the definitions of ATP Ⅲ, CDS and IDF. The prevalence of MS per IDF is the highest among three definitions.
