摘要 目的探讨CO_2激光、Nd:YAG激光和半导体激光治疗颅内肿瘤的效果及适应证。方法对1996年以来应用CO_2激光、Nd:YAG激光和半导体激光治疗的51例脑肿瘤患者进行回顾性分析。结果CO_2激光气化垂体瘤6例,听神经鞘瘤4例,胶质瘤8例和脑膜瘤4例,均达到切除肿瘤的目的。Nd:YAG激光气化脑膜瘤19例,胶质瘤7例亦均达到切除肿瘤的目的。半导体激光间质内热疗垂体瘤3例,效果显著。结论CO_2激光、Nd:YAG激光均能达到气化脑肿瘤的目的,但均存在气化时间长,粉尘多的缺点。Nd:YAG激光气化肿瘤的止血效果优于CO_2激光,CO_2激光较适于重要部位残留的较薄肿瘤的全切。半导体激光用于垂体瘤间质内热疗,效果较佳。 Objective To explore the effect and indication of CO2 laser, Nd : YAG laser and semiconductor laser in dealing with intracranial tumors. Methods Fifty one patients with brain tumors who had been treated surgically with CO2 laser, Nd:YAG laser or semiconductor laser from 1996 were analyzed retrospectively. Results In these patients, 6 with pituitary tumors, 4 with acoustic neurinomas, 8 with gliomas and 4 with meningiomas, had tumors effectively vaporized by CO2 laser; 19 with meningiomas and 7 with gliomas had their tumors effectively vaporized by Nd :YAG laser; Furthermore, 3 with pituitary tumors were treated with interstitial semiconductor laser thermotherapy and the curative effect was satisfactory. Conclusions CO2 laser and Nd : YAG laser are both effective in treating with brain tumors, but they need much time and make much dirt; Nd : YAG laser is superior to CO2 laser on hemostatic effect and CO2 laser is more suitable for the complete resection of the remnant thin tumors in the important anatomic sites. Semiconductor laser is fit to the interstitial laser thermotherapy(ILTT) of pituitary tumors and the therapeutic efficacy is fine.
