摘要
目的探讨足背浅静脉营养血管远端蒂筋膜皮瓣移植修复足趾电烧伤创面的临床效果。方法选择2006年1月-2007年12月由我科采用足背浅静脉营养血管远端蒂筋膜皮瓣移植修复足趾电烧伤4例。其中皮瓣修复大趾刨面2例,修复小趾刨面2例。术后定期随访,观察足背浅静脉营养血管远端蒂筋膜皮瓣移植修复术后的存活情况。结果4例患者中均有轻度的皮瓣肿胀,其中,1例皮瓣远端少量水泡,1例皮瓣远侧边缘少量坏死,经换药愈合;伤口I期愈合3例,Ⅱ期愈合1例。所有病例经1~3个月随访,刨面修复,外形满意。结论足背浅静脉营养血管远端蒂筋膜皮瓣移植修复足趾电烧伤刨面,具有手术简便,不损害主十动脉以及成活率高等优点。
Objective To investigate the clinical effect of distally based fasciocutaneous flap pedicled with nutrient vessels of the dorsal cutaneous vein of foot on repairing the toe injury from electric shock. Methods Investigated 4 cases of the toe injury from electric shock repaired with distally based fasciocutaneous flap pedicled with nutrient vessels of the dorsal cutaneous vein of foot that happened between Jan. 2006 to Dec. 2007. Of them, 2 cases are the first toe and another two cases are the fifth toe. All patients were followed up post - operation to observe the sruvival condition after surgery. Results The.flaps in 4 cases were found swelling at various degrees post - operation. Of them, in 1 case some blisters were found at the far end edge ; and in another case the flap was necrosis on its distal margins, both cases were healed by repeated dressing changes. Overall, 1 st stage healing were found in 3cases, while one case in 2nd stage healing, All the patients were followed - up for 1 to 3 months, and all cases showed satisfactory appearance on repaired wound surfaces. Conclusion Distally based fasciocutaneous flap pedicled with with nutrient vessels of the dorsal cutaneous vein of foot can be used to repair toe tissue defects caused by injury from electric shock with the advantages of rapidness, convenience, without damaging main artery on foot and has very high survival rate.
