球囊环代替琼脂环制作食管静脉曲张犬模型

Establishment of esophageal varices canine model by balloon dilatation constrictor replacement of ameroid constrictor

摘要 目的改进制作食管静脉曲张犬模型的方法。方法选取成年杂种犬22条,采用可充式球囊缩窄环渐进性压迫缩窄门静脉,联合门腔转流方法(以下简称可充式球囊缩窄造模法)制作食管静脉曲张犬模型.可充式球囊缩窄环由透明PP料环、带球囊硅胶管、植入式输液港中的注射座(硅胶)三部分组成。成模前后通过门静脉压力测量、胃镜检查、门腔静脉系统造影评价造模效果。结果造模前门静脉平均压力为(12.864-0.18)cmH2O,成模后门静脉平均压力为(26.75±0.39)cmH2O(P〈0.05);成模后胃镜下见食管静脉曲张程度不等,胃镜分级I级4条、Ⅱ级6条、Ⅲ级8条、Ⅳ级4条;门腔静脉造影显示下腔、门静脉完全被阻断,吻合口通畅,食管、胃底曲张静脉显影。结论可充式球囊缩窄造模法是制作食管静脉曲张犬模型的有效方法。 Objective To improve the method to establish esophageal varices canine model. Methods On selected 22 adult mongrel dogs, balloon dilatation constrictor was used to squeeze portal vein gradually,with side-to-side portacaval shunt and inferior vena cava ligation, and canine model of esophageal varices was established. Balloon dilatation constrictor was composed of hyaline polypropylene ring, the silica gel tube with balloon,vascular access device port. The models were evaluated through pre-and postshunt portal pressure measurement,weekly endoscopies and portacaval angiography. Results The mean ( +/-SE) pre- and postshunt portal pressure was increased significantly from( 13.3±1.5 )to(25.6±2.2)cm H″ 2O (P 〈 0.05 ). Endoscopy was performed to grade the varix size, and it was found there were 4 cases of esophageal varices in class I ,6 in grade Ⅱ ,8 in grade Ⅲ ,and 8 in grade Ⅳ ,respectively. Portacaval angiography showed that inferior vena cava and portal vein were completely blocked, anasto- motic stoma was unobstructed, and shunting of blood through esophageal varices via splenic and gastric veins was visualized. Conclusion Balloon dilatation constrictor modeling is an effective method for establishment of esophageal varices canine model.
