手术治疗颞下颌关节盘穿孔的临床研究 被引量:7

A clinical study of surgical treatment with temporomandibular joint disc perforation

摘要 目的探讨手术治疗颞下颌关节盘穿孔的疗效。方法对接受手术治疗颞下颌关节盘穿孔的患者进行术后随访,共67例(83侧关节)患者具备完整的资料,比较术前、术后3、6、12、24及24个月以上疼痛、弹响及开口度变化。结果手术后关节盘穿孔患者术后3、6、12、24及24个月以上的疼痛改善依次为73.7%、80.0%、86.2%、83.3%、77.8%,关节杂音改善依次为80.0%、100.0%、58.8%、57.1%、77.8%,最大张口度依次为31、35、35、35、38mm。手术后6个月时症状均有明显改善。结论手术治疗对于颞下颌关节盘穿孔具有显著疗效。 Objectives To evaluate the clinical outcome of surgical treatment with temporomandibular joint disc perforation. Methods Follow-up was done on 191 patients who had received surgical treatment on temporomandibular joint disc perforation from Hospital of Stomatology, Wuhan University. 67 patients had complete documents on signs of joint pain, joints sounds and maximal mouth opening before surgery and in 3, 6, 12, 24 months after surgery and above respectively. Results Pre-operative signs were improved significantly after 6 months of the operation. The relieving rate of post-operative joint pain was 73.7%, 80.0%, 86.2%, 83.3%, 77.8% in 3, 6, 12, 24 months after surgery and above respectively, while the improving rates of joints sounds were 80.0%, 100.0%, 58.8%, 57.1% and 77.8%. For the maximal mouth opening, it was 31, 35, 35, 35 and 38 mm respectively. Condusion Operation is effective for temporomandibular joint disc perforation.
