肾组织炎症细胞浸润与其类肝素酶高表达相关 被引量:3

摘要 目的观察进展期慢性肾炎患者外周血淋巴细胞及肾组织类肝素酶(heparanase,Hpa)基因表达情况,并初步探讨Hpa与肾组织炎细胞浸润的相关性。方法进展期慢性肾炎患者30例及肾移植健康供者10例,采用RT-PCR方法检测外周血淋巴细胞类肝素酶mRNA表达,肾组织石蜡切片Hpa免疫组化染色检测肾组织Hpa的表达,并计数比较肾组织中炎细胞浸润程度。结果慢性肾炎患者外周血淋巴细胞HpamRNA含量明显高于正常人(P<0.05);免疫组化显示,慢性肾炎患者肾小球及纤维化小管间质中浸润炎细胞也存在Hpa表达,而正常供肾仅见肾小球内Hpa极低表达(P<0.05),Hpa表达随肾组织炎细胞浸润程度加重而递增。结论慢性肾炎患者外周血淋巴细胞存在Hpa高表达,肾组织炎细胞浸润与Hpa的高表达存在一定相关性。 Objective To observe the expression of heparanase (Hpa) in the peripheral lymphocytes and renal tissue of patients with progressive chronic nephritis, and to investigate the correlation between the expressions and inflammatory cell infiltration. Methods Totally 30 cases with chronic nephritis in progression and 10 individuals as healthy donors for kidney transplantation were subjected. RT-PCR was used to determine the Hpa mRNA expression in peripheral blood lymphocytes, and immunohistochemical staining was used to paraffin sections of renal tissue to determine Hpa expression. The inflammatory cell infiltration was observed in renal tissue. Results Hpa mRNA expression in peripheral blood lymphocytes was significantly higher in chronic nephritis than normal control ( P 〈 0.05 ). Immunohistochemical staining indicated that there was Hpa expression in infiltrated inflammatory cells in glomerulus and fibrotic tubule stroma of chronic nephritis, while only very low Hpa expression was found in glomerulus of normal donor (P 〈 0.05 ). Therefore, Hpa expression became stronger with more inflammatory cells infiltrating to renal tissue. Conclusion There is high Hpa expression in peripheral blood lymphocytes of chronic nephritis, and there might be a positive correlation between inflammatory cell infiltration and high Hpa expression in renal tissue.
