摘要
目的:进行臂丛神经根切断对肢体功能影响的实验研究,并探讨其在治疗痉挛性脑瘫中的应用。方法:SD大鼠24只,分为3个实验组,各组8只,每组分别切断左侧1个神经根(C_6,C_7,C_8)。左侧神经根切断后,立即对双侧三角肌、肱二头肌、肱三头肌和前臂屈肌群进行肌电检测,并对运动诱发电位波幅进行比较分析。结果:切断C_(6~8)神经根中任何一根对大鼠肢体功能均无影响。临床应用C_7神经根切断治疗脑瘫所致上肢痉挛1例,取得满意的近期疗效。结论:采用单根神经根切断治疗痉挛性脑瘫,是值得研究的一种新方法。
Objective: To study the influence of rhizotomy of brachial plexus on the function of upper limbs in an experiment model and investigate its application in the treatment of cerebral palsy spasticity. Methods: 24 SD rats were divided into 3 groups, with 8 in each. C_6C_7 and Cs nerve root of left brachial plexus was cut respectively. After rhizotomy, bilateral electromyographic examination was conducted in deltoid, triceps brachialis, biceps brachialis and flexor muscles of the forearm. Amplitude of evoked motor action potentials of the corresponding muscles were compared. Results: In rat experiment model,the function of the limb was not affected after rhizotomy in either of the C_(6-8) nerve roots. The application of rhizotomy in a clinical case of cerebral palsy spasticity of the upper extremity yielded satisfying short term result. Conclusion: Selective rhizotomy of brachial plexus for treatment of cerebral palsy spasticity is a new method of potential value.
