摘要
【目的】观察不同类型冠心痛(CHD)患者血浆白介素18(IL-18)、白介素10(IL—10)水平及其比值的变化,探讨IL-18和IL-10在CHD发生发展中的可能作用。【方法】把96例CHD患者分为A组(Gensini积分〈26分)、B组(Gensini积分26~54分)和C组(Gensini积分〉54分),采用酶联免疫吸附法检测不同类型CHD患者血浆IL-18、IL-10的浓度变化。【结果】CHD患者血浆IL-18、IL-10水平及其比值明显高于对照组;急性心肌梗死患者血浆IL-18的水平及IL-18/IL-10显著高于不稳定型心绞痛及稳定型心绞痛病人,不稳定型心绞痛患者高于稳定型心绞痛患者(P〈0.01);急性心肌梗死和不稳定型心绞痛患者血浆IL-10水平明显低于稳定型心绞痛患者(P〈0.01);C组血浆IL-18的水平及IL-18/IL-10明显高于B组及A组,B组显著高于A组(P〈0.01)。【结论】CHD病人血浆IL-18及IL-18/IL-10明显升高,且在一定程度上反映了病情、病变的严重程度,可作为CHD危险分层及病变程度评估的参考指标;IL-18/IL-10失调与CHD的急性加重相关。
[Objective]To investigate the changes of interleukin-18(IL-18), interleukin-10(IL-10) and IL- 18/IL-10 ratio in patients with coronary heart disease(CHD), and to explore their contributions in the development of CHD. [Methods]Ninety six patients with CHD were divided into three groups: group A (Gensini scoreG26) , group B (Gensini score between 26-54) and group C (Gensini score〉54). The concentrations of plasma IL-18 and IL-10 in patients were measured by enzyme linked immunosorbent assay (ELISA). [Results]The concentrations of plasma IL-18, IL-10 and IL-18/IL-10 ratio in the group of CHD were higher than those in thecontrol group, The concentration of plasma IL-18 and IL-18/IL-10 ratio in the patients with acute myocardial infarction (AMI) were higher than those in the patients with unstable angina pectoris (UAP) and stable angina pectoris (SAP), and those in the patients with UAP were higher than those in the patients with SAP( P 〈0.01). The concentration of plasma IL-10 in the patients with UAP and AMI were higher than those in patients with SAP ( P 〈0.01). The concentration of plasma IL-18 and IL-18/IL-10 ratio in group C were higher than those in group A and group B, and those in group B were higher than those in group A( P 〈 0.01). [Conclusion]The concentration of plasma IL-18 and IL-18/IL-10 ratio in the group of CHD are higher than those in the control group, and to a certain extent, they may reflect the severity of the illness and be used as reference indexes for risk stratification and severity evaluation of CHD. The imbalance of IL-18/IL-10 is probably relative to the development of CHD.
