即刻加载后牙一期法牙种植术

Clinical evaluation for immediate loading on posterior implants by one stage.

摘要 目的:探讨后牙即刻加载种植手术适应征、操作技巧,评价其远期临床疗效。方法:CDICI一期法柱状种植体植入术修复后牙缺失,术后1月内完成上部结构修复,通过检测种植体动度,GI及种植体周围炎、X线口腔全景片等临床指标判断种植义齿成功率。结果:10例患者、共28个种植体术后随访1年,均获临床成功。结论:手术适应症应为牙槽嵴丰满之青壮年患者;种植体周围骨壁应>2mm;环切打孔式非翻瓣切口,有利于黏骨膜瓣创口愈合,减少种植体周围炎发生的可能性;制备窝洞直径应小于种植体直径0.2~0.5mm,以保证其良好的稳定性;螺纹状种植体能增加种植体接触面积和镶嵌能力,有宜于传递和分散力。 Objective: Adaptation and surgical skills had been discussed and long clinical functions were evaluated. Method-CDIC one-stage implants were used in posterior regions. After one month, supper structrures were restored for implants primany stability, GI, peri-implantitis, x-ray films were analysed to determine the successful rates.Result: After oneyear follow-up, 10 cases with 28 dental implants were success.Conclusion: Immediate loading technique is suitable for patients with adequate bone in posterior region. The bone amount around implant should be more than 2 mm. The diameter of surgical hole must be less 0.2-0.5 mm than implant in order to get primary stability. The screw surface of CDIC implant is benefit for increasing contact area, decreasing biting force.
