经胸超声心动图对房间隔缺损封堵术失败原因分析 被引量:2

摘要 目的评价经胸超声心动图(TIE)在房间隔缺损(ASD)封堵术中的应用价值。方法应用TIE术前检查继发ASD患者49例,确定ASD大小、形态及缺损周边残存房间隔的长度,观测有无分流及瓣膜异常。结果41例患者封堵成功,ASD大小平均为(16.5±6.3)mm,封堵器大小平均为(18.5±8.3)mm,TTE测量大小与封堵器大小相关良好(P<0.001),8例患者封堵失败。结论TIE通过剑下双房矢状切面、大血管短轴切面及心尖四腔切面观察ASD及其周边结构,即可确保封堵术的成功。 Objective To evaluate the transthoracic echocardiography (TIE) in transcatheter occlusion of atrial septal defect and analysis of the reasons for the failure. Methods Before occlusion, the size, shape of ASD and the remaining atrial septum length were measured, the shunt was observed by color Doppler flow imaging(CDFI). Results Forty - nine patents with ASD were occluded, including 8 failed. Size of ASD were 4 - 38( 16.5 ± 6.3)mm, and occluders size were 8 - 42(18.5 ± 8.3)mm. Close correlation was found between the size of ASD measured by TIE and the size of occluders ( P 〈 0. 001 ). Conclusion TIE would be is a feasible, simple and effective method in ASD occlusion, subcostal two- antrum view, the short- axis view of great vessels and apical fourchamber view could clear reveal ASD to ensure successful occlusion.
