摘要
目的评价经胸超声心动图（TTE）指导硬缘房间隔缺损（ASD）封堵术的可行性和作用。方法298例硬缘ASD患者，术前用彩色多普勒超声心动图结合组织谐波功能经胸测量各切面ASD大小及周缘情况，ASD封堵术中TTE监测封堵过程、封堵器的牢固性，彩色多普勒观察有无残余分流。封堵器释放后测量其腰部大小，并与术前TTE测量的ASD最大径进行比较。结果用TTE术前检查ASD最大径3～40mm[平均（20．0±8．5）mm]，选用的封堵器6～46mmP平均（24．9±9．4）mm]，释放后封堵器腰径3～40mmP平均（20．5±8．6）mm]。TTE测量的ASD大小与封堵器大小相关良好（Y=1．11X＋2．67，r=0．98，P〈0．0001）。TTE测量的ASD大小与释放后封堵器腰径相关关系极佳（Y=0．99X＋0．24，r=0．99，P〈0．0001）。所有患者ASD封堵器固定牢固，无脱落。结论TTE结合组织谐波功能可以准确测量硬缘ASD大小，是指导ASD封堵的可行、有效、简便方法。
Objective To evaluate the feasibility and effect of transthoracic echocardiography（TTE） on guiding the occlusion of the hard-rim atrial septal defect（ASD）. Methods ASD size and its rim of two hundred and ninety eight patients with the hard-rim ASD were measured by TTE on various views with color Doppler system and tissue harmonic function before occlusion. Occluding process and steadiness of occluder were monitored by TTE,and whether residual shunt was left was detected by color Doppler imaging during occlusion. The waist size of occluder was measured after releasing occluder and compared with the longest diameter of ASD measured by TTE. Results The size of ASD measured by TTE before occlusion was 3-40 mm[（20.0 ± 8.5）mm], the size of selected occluder was 6-46 mm[（24.9 ± 9.4）mm] and the waist size of released occluder was 3-40 mm [（20. 5 ± 8.6）mm]. Fine correlation was existed between the size of ASD measured by TTE and the size of selected occluder （ Y = 1.11 X ＋ 2.67, r = 0. 98, P 〈0. 0001）. Excellent correlation was found between the size of ASD measured by TTE and the waist size of released occluder（ Y = 0.99 X ＋ 0.24, r = 0.99, P 〈0. 0001）. Occluder was firmly fixed without falling of in all patients. Conclusions TTE with tissue harmonic function can be used to accurately measure the size of hard-rim ASD and is a feasible,effective and simple method for guiding ASD occlusion.
