摘要
目的:探讨BPH的MSCT多期扫描不同时期的成像特点和规律以及与MVD、VEGF表达情况的相关性。方法:23例BPH患者经手术病理证实且术前行MSCT多期扫描。病理学标本分别进行HE染色和VEGF、CD34的免疫组化SP染色。对照分析MVD、VEGF的表达情况与BPH的MSCT多期增强扫描不同时期的成像特点的关系。结果:BPH中央区在25s有轻度强化,40s、55s、70s、180s强化明显,中央区在平扫和第25s、70s和180s增强扫描时CT值差别无统计学意义(P>0.05),其它各扫描时期CT值两两比较其差别有统计学意义(P<0.01)。周边区在25s有轻度强化,40s、55s、70s、180s强化明显,周边区CT值差别在平扫和第25s增强扫描时无统计学意义(P>0.05),其它各扫描时期两两比较CT值差别有统计学意义(P<0.01)。中央区和周边区CT值差别在第40s增强扫描后均有统计学意义(P<0.01)。BPH中央区平均MVD值为20.39±6.45,周边区平均MVD值为7.45±4.34,二者的差别有统计学意义(P<0.01);中央区VEGF表达阳性率为54.35%,周边区VEGF表达阳性率为13.48%,二者的差别有统计学意义(P<0.01)。BPH中VEGF表达强度与MVD值之间呈正相关(r=0.8634,P<0.05)。MVD值与前列腺体积无相关性(r=-0.206,P>0.05)。CT强化幅度随VEGF表达水平和MVD值的增高有增加的趋势。结论:MSCT强化幅度在一定程度上可反映BPH内微血管的形成和VEGF表达情况。
Objective: To investigate the correlations of the expressions of microvascular density (MVD) and vascular endothelial growth factor (VEGF) with the MSCT features of benign prostate hyperplasia (BPH).Methods: Twenty-three BPH cases proved by histopathology were examined with multi-phase contrast-enhanced MSCT scanning before operation or biopsy. Each pathologic specimen was stained with hemetoxylin and eosin and SP immunohistochemical technique. The CT features and the expressions of MVD, VEGF were analyzed to investigate the relationship with each other.Results:There was slight enhancement in the CZ (central zone) at 25 s, significant enhancement at 40 s, 55 s, 70 s and 180 s. There was no difference between the CT values of plain scan and 25 s, 70 s and 180 s ( P 〉 0.05). The difference of the CT values of between the other phases was significant ( P 〈 0.01). There was slight enhancement in the PZ (peripheral zone) at 25 s, significant enhancement at 40 s, 55 s, 70 s and 180 s. Except the plain scan and 25 s, there was difference between the CT values of different phases ( P 〈 0.01 ). The CT value of the CZ was higher than that of PZ after the 40s ( P 〈 0.01 ). The mean MVD of CZ ( 20.39 ± 6.45 ) was higher than that of PZ ( 7.45 ± 4.34 ) ( P 〈 0.01 ). The general positive rates of VEGF of CZ and PZ were 54.35% and 13.48% respectively ( P 〈 0.01). There was significandy positive correlation between VEGF expression and mean MVD ( r = 0.8634, P 〈 0.05). There was no correlation between MVD and the volume of prostate. The CT enhancement degree increased with the higher expressions of VEGF and MVD value according to the results of stepwise regression ( P 〈 0.05). Condusion: The degree of enhancement of MSCT can be regarded as an effective method to evaluate the VEGF expression and angiogenesis of BPH.
