In human heart,alphal ,betal and beta2 adrenceptors(AR)exert a powerful inotropic and chronotropic effect in both physiologic and pathologic conditions to response for the sympathetic stimulation.Of the changes in myocardial adrenoceptors,beta AR phenomena are the most striking and down regulation is the most prominent.Desensitization of beta AR/G protein/adenylyl cyclase includs receptor downregulation,uncopling and activity of Gi increased those come from chronic exposure of beta receptor agonists consistently,meanwhile proportion of beta2 and alphal receptor is much higher in failing heart.Taken together,clinic practice that antiadrenergic therapy would be effective in the treatment of heart failure by removihg adrenergic signaling transdused by the remaining components of the receptor pathways and also the receptor desensitization changes present in the failing heart provide a rationale for betal plus beta2 alphal adrenergic receptor blockade in heat failure。
