摘要
目的探讨肺硬化性血管瘤(sclerosing hemangioma of Lung,SHL)组织成分、免疫组织化学、病理形态学特征、免疫组化在诊断中的价值。方法用免疫组化SP法对3例SHL组织标本的表面活性蛋白-B(surfactant protein-B,SP-B)、LCK、Vim、NSE、CEA、EMA、CD34、SMA、MC、CgA、ER、VEGF进行标记,观察其表达情况。结果SHL主要表现为乳头区、实性细胞区、血管瘤样区及硬化区等四种结构,免疫组化显示乳头、实性区裂隙及血管瘤样区表面衬覆细胞阳性表达SP-B、LCK、CEA、EMA,圆形或多角形细胞阳性表达Vim和CgA,NSE和EMA在圆形或多角形细胞灶性表达阳性,CD34仅在肿瘤组织中的血管内皮细胞呈阳性表达,MC和SMA在两种细胞均为阴性表达,ER在部分圆形或多角形细胞弱阳性表达,其中一例乳头表面衬覆的立方状上皮细胞ER弱阳性表达,VEGF在两种细胞均为阳性表达。结论SHL的组织成分包括表面的立方状上皮细胞、圆形多角形胞质浅染的细胞、肺泡腔内出血和硬化性变化,四种组织结构至少存在三种,免疫组化对SHL的诊断有帮助。ER及VEGF在肿瘤细胞的表达可能可以为SHL的临床治疗提供新思路。
Objective To analyze composition and immunohistochemistry of SHL, and to investigate pathological characteristics and diagnostic value of immunohistochemistry of SI-IL. Methods Using immunohistochemistry methods, the expression of SP-B, LCK, Vim, NSE, CEA, EMA, CD34, SMA, MC, CgA, ER and VEGF were observed in 3 cases of SI-IL. Results SHL demonstrated the histologic patterns with variable proportions of solid, papillary, hemanggionma-like and sclerotic pattern. Cuboidal or colummar cells covered the papillary and cleft-slide spaces in solid areas whereas solid areas and stalk of papillary projections were composed of uniform, round or polygonal pale cells. The cuboidal or colummar cells were positive for SP-B, LCK, CEA, EMA. The round or polygonal pale cells expressed vimentin and CgA, with some of them showing positive for NSE and EMA. Only endothelial cells of blood vessel were positive for CD34. Both cuboidal or colummar and round Or polygonal pale cells displayed negative immunohistochemical result to MC and SMA. Some of the round or polygonal pale cells expressed ER. One of the three cases of the cuboidal or colummar cells was positive for ER. And the two kinds of cells were positive for VEGF. Conclusion The SHL consists of cuboidal cells and polygonal pale cells and hemanggionma - like areas and sclerotic areas. The presence of three patterns is necessary. The immunohistochemistry is helpful to the diagnosis. Expression of ER and VEGF maybe give a new way to treating SHL.
