实践糖尿病患者健康教育服务流程提高治疗依从性 被引量:3

摘要 目的研究与实践糖尿病(DM)患者的门诊-住院-出院健康教育服务流程,从而提高患者治疗依从性。方法①门诊教育:糖尿病护理门诊、糖尿病学校集体授课与讨论交流形式相结合、健康论坛在线问答;②住院教育:以小组授课为主,结合床旁单人指导和自学DM知识宣传资料、小组成员交流;③出院后教育:以医患沟通室电话随访咨询方式。结果住院患者在教育前DM知识测试的合格率为20.8%,教育培训后达到80.5%(P<0.001)。培训后患者进行自我血糖、自我足部检查护理人数均明显增多(P<0.005),胰岛素治疗、用药治疗依从性明显提高(P<0.005)。结论实践糖尿病(DM)患者的门诊-住院-出院健康教育服务流程,提高了患者自我管理能力和治疗的依从性。 Objective To study and practise service procedure of diabetes health education, improve compliance of treatment, nehtods We established groups for diabetes education located in DM nursing clinic, DM educational school and internet outpatient education included lectures and discussions. Inpatient education included group education, individual education and self education, posthospital education inchlded consulting online. Results Among hospitalized patients, the passrate of knowledge tests was 80. 5% alter education, compared with 20. 8% before education (P 〈0. 001 ) . Knowledge on self care skills such as self monitoring of blood glucose, and footcare, were improved ( P 〈 0. 005 ) , compliance of treatment was improved. Conclusion Practising service procedure of diabetes education provide diabetic knowledge for patients with diabetes, improving the ability of self managemen and improving compliance of treatment.
